The goal is to evaluate the effectiveness of the developed technique of the gradual forming of the internal limiting membrane (IML) fragment in the treatment of large macular holes (MH) in comparison with the standard methodol-ogy (using classical maculorhexis and rapprochement of MH edges by using of vacuum aspiration). Materials and methods: we observed 137patients (103women, 34men) aged from 54 to 78years with large MH (over 400μm). Duration of MH ranged from 3 to 60months. All the patients were divided into groups: 1 (new methodology) and 2 (standard method), and sub-groups: 1a and 2a – patients with MH with a minimum diameter 400–650μm; 1b and 2b – patients with MH with a minimum diameter more than 650μm. The new technique is performed in several stages, during which several series of ILM “petals” removal are made, leaving an intact area on the MH edge. The last ILM “petal” or fragment is inverted and placed on the MH. Results: In subgroup 1a, there was a statistically significant increase in best corrected visual acuity (BCVA) during all follow-up with the most pronounced growth in the first 6 months (from 0.15±0.06 to 0.55±0.14) (p<0.05). In subgroup 1b a statistically significant increase in mean values of BCVA was also observed especially during first 6month (from 0.13±0.06 to 0.36±0.12) (p<0.05). In group2 (a, b) BCVA was significantly lower (p<0.05). Conclusion: the new technique allows to reach higher functional-anatomical results in surgical treatment of MH.
Dorsopathies are enduring importance among medical problems. This confirms the fact that the World Health Organization declared a theme associated with dorsopathies, as priorities for the next five-year period. It should be emphasized that the etiology and pathogenesis of these states are reviewed with the «enviable» frequency, and this, naturally, is reflected on the principles of treatment and rehabilitation of this contingent. In this review the mechanisms and features of application of the basic drug and non-drug methods of treatment used in modern clinic, are considered. It should be noted that a use of a complex approach, aimed at correction and relief of various vertebral and pathological manifestations, is optimal in this case. In particular, the review presents in details the options for medical treatment and a variety of technologies of physical stimulation - from traction to modern hardware methods of physiotherapy. Taking into account authors’ own original developments, special attention is paid to theoretical and practical aspects of reflexology, which is widely used in complex therapy of patients with dorsopathies. In this block, the authors provide Oriental, traditional approaches to acupuncture and promising ways, based on the combined use of Eastern and Western medical schemes. The authors identify separately the pharmacopuncture as the action caused by the summation of the effects as reflex irritation and entered in the field point of medicaments. Pharmacopuncture involves the use of various medicines, but complex homeopathic remedies provide a distinct therapeutic result, i.e., the authors suggest that there are subtle mechanisms of interaction between point and drugs of this class.
The article presents modern concepts of “cardiovascular continuum”, where there are stages of heart disease risk factors up to death. The fundamental constant is a magnesium deficiency as a risk factor for endothelial dysfunction. The authors show the way of absorption, distribution of the elimination of magnesium, the regulatory significance of genes, providing its bioavailability. Symptoms of magnesium deficiency on the part of the cardiovascular system are characterized. The authors specify the involvement of magnesium in the develop-ment of metabiological syndrome, the possibilities of correction of its content, respectively, and insulin resis-tance. An anti-arrhythmogenic effect of the drug magnesium – Magnerot - was proven.
The article presents the data of scientific medical literature about the relationship of the oral cavity microbiota and gastrointestinal tract with atherogenic dyslipidemia and its role in the development of cardiovascular complications. The article shows the results of epidemiological studies of association level of inflammatory diseases in the oral cavity with atherogenic diseases of cardiovascular continuum. The mechanisms of periodontal microflora and its role in the formation of unstable atheromatous plaques in the vascular channel with possible clinical manifestations (ACS - MI, cerebral stroke) are discussed. The authors give their data of retrospective analysis 1000 outpatient’s cards of periodontitis patients of varying severity, depending on BMI and their relationship to the levels of carbohydrate\lipid metabolism, state of systemic inflammation and oxidative stress.
The article presents a retrospective analysis of 1000 medical records of patients suffering from CGP varying of different degree of severity, which were divided into two subgroups: BMI≥25 kg\m2 (572 patients), and BMI<25 kg\m2 (428 patients). A retrospective analysis has demenstrated a direct correlation of severity and frequency of the combination of CGP somatic disease (CVD, type 2 diabetes, diseases of the lungs and HKT ) in patients with a BMI≥25 kg/m2. The authors also presented a correlation analysis of the relationship of gravity CGP with biochemical status in 73 patients with CGP and BMI≥25 kg\m2, which conducted the study lipid - carbohydrate spectra of blood, activity state of systemic inflammation and oxidative status, the biomass of the bacterial flora of the mouth and intestines before and after treatment with the added attraction of probiotics (Simbiolakt comp.) and antioxidants (Ubiquinone comp.). The revealed high direct correlations bleeding index and severity of CGP (rIK - StHGP=0.89), BMI and bleeding index (rIMT - IR=0.79), atherogenic plasma (IA) and CGP (rIK -NA=0.72), pro-inflammatory status and BMI (rIMT - MDA=0.87) allowed the authors to conclude - CGP varying severity is independent risk factor for CVD in patients with a BMI≥25 kg\m2.
