99m-Technetium sestamibi uptake in a gastric schwannoma

Abstract: We report the case of a 74-year-old woman with primary hyperparathyroidism who underwent 99m-technetium-sestamibi single photon emission computed tomography-computed tomography for preoperative localization of parathyroid adenoma. Unexpected focal sestamibi uptake was observed at a 5 cm submucosal tumor arising from the greater curve of the stomach. The patient underwent partial gastrectomy and the histological and immunohistochemical findings were consistent with the diagnosis of gastric schwannoma.

“…Outside the neck, uptake of 99m Tc-sestamibi in schwannomas has already been described as a rare finding in the head ( 2 ), and to some extent it has been shown to be an effective radionuclide in the localization of primary and metastatic brain tumors and acoustic schwannomas ( 99m Tc-sestamibi brain SPECT of cerebellopontine angle tumors). It has also been reported as an incidental finding, identifying a gastric schwannoma in a patient investigated for primary hyperparathyroidism ( 23 ).…”

“…In another study, technetium-99m sestamibi single-photon emission CT/CT was also found to be conducive to achieving accurate anatomical localization. 27 Immunohistochemistry is of major significance to the differential diagnosis among GISTs, leiomyomas, and gastrointestinal autonomic nerve tumors. Schwannomas are positive for S100 protein and vimentin but negative for CD34 and CD117, which is in stark contrast to GISTs.…”
