A 30-month prospective study on the treatment of retinoblastoma in the Gabriel Toure Teaching Hospital, Bamako, Mali

Abstract: Mortality as a result of retinoblastoma remains high in our region because diagnosis is always late and it is difficult to follow-up the patients. Education of healthcare workers and raising awareness in the general population would improve the survival rate of retinoblastoma patients in Africa.

“…This pattern of parents not accepting the enucleation of the affected eye or defaulting from follow-up after a conservative intent is similar in many areas of the developing world. Rates from 18% to 50% have been reported by groups in from Asia, Africa, and Central America [11,13,21,26,29,33]. Decisions against enucleating the second affected eye after conservative treatment fails account for a survival rate of less than 60% in patients with bilateral retinoblastoma in Taiwan, a relatively affluent country having a national healthcare program that offers subsidized care [34].…”

“…In contrast, in most MICs, metastatic disease is present at diagnosis in less than 10% of the cases [7,22,23]. Treatment with neoadjuvant chemotherapy, followed by secondary enucleation and adjuvant therapy is the current approach in most centers [13,40,41]. Encouraging results have been reported for the treatment of retinoblastoma that is disseminated to the orbit or the preauricular lymph nodes has resulted in disease-free survival rates over 70% [41].…”

“…On the other extreme, presentations of retinoblastoma differ. From Sudan 9 , buphthalmos (56%) and leukocoria (32%) were most common, and from Mali in Africa 10 , proptosis (55%), leukocoria (38%), strabismus (6%), and buphthalmos (2%) were found. Efforts are underway internationally to educate clinicians, nurses, patients, and entire populations for improvement in retinoblastoma detection.…”

“…Enucleation continues to remain critical for advanced retinoblastoma, particularly in Asia and Africa. 3,10,11 In the 1980s, external beam radiotherapy (EBRT) was popular, but later-realized risks of radiationrelated second cancers have lead to reduction in use of this modality. In the 1990s, systemic IVC was introduced with agents of vincristine, etoposide, and carboplatin (VEC).…”
