A 50-year experience with autoimmune hepatitis: and where are we now?

Abstract: The challenge remains with both types of AIH to elucidate in molecular terms the genetic and environmental basis of pathogenesis from initiation to ultimate progression and cirrhosis (when inadequately treated). Advancing technologies are bringing this goal closer to being attainable.

“…The liver lesions of “classic” SLE are relatively trivial compared with those of autoimmune “lupoid” hepatitis (2, 14). However, cases of lupoid hepatitis have been reported in SLE patients, suggesting that an overlap syndrome of SLE-AIH may exist (13).…”

“…For an extensive review of the history of lupoid hepatitis, see (2). The initial patients in Kunkel’s case series, predominantly young women, had a history suggestive of acute hepatitis and developed cirrhosis (1).…”

“…Our attempts to find in available databases the linkage between an appearance of autoantibodies to these enzymes and specific human autoimmune diseases or reproductive dysfunctions in women have failed. A link between actin autoantibodies and autoimmune diseases is well known, and it was found in patients with autoimmune liver diseases (12,13), celiac disease (14), and rheumatoid arthritis (15). The endoplasmin also known as Heat Shock Protein 90 kDa Beta Member 1 may participate in immune response in RM patients.…”

“…Notwithstanding, there is a near invariable presence in serum of one or another of several relatively disease-specific autoantibody reactants, albeit demonstrable at various levels of sensitivity and specificity. Detailed discussion of these and other clinical issues are beyond the scope of this review (see Mackay, 2011aMackay, , 2011b. The present enquiry is pitched more towards the origin and meaning of these abnormal reactants.…”

“…Other autoantibody specificities recognized in AIH are described elsewhere (Bogdanos et al, 2008;Mackay, 2011aMackay, , 2011b. Included are SLA/LP, molecularly identified as selenocysteine synthase, and liver cytosol type 1 (LC1).…”
