A Case of Optic Neuritis Complicating Herpes Zoster Ophthalmicus in a Child

Abstract: Here we report a case of optic neuritis in the setting of herpes zoster ophthalmicus (HZO) in a child. A six-year-old girl presented with HZO in the right eye. During the hospitalization, her visual acuity decreased. Fluorescein angiography (FAG) and optical coherence tomography revealed optic neuritis in the affected eye. Visual acuity improved with one month of treatment with acyclovir and steroids. FAG analysis showed no evidence of leakage at the optic disc. At one year post treatment, the patient's fundus… Show more

“…Lymphocytic infiltration occurs along the posterior ciliary vessels and nerves causing occlusive vasculitis and ischaemic injury 11. An alternative mechanism of injury can be caused by lymphocytic infiltration of the orbital nerves via sensory offshoots of trigeminal nerve to the motor cranial nerves 12.…”

“… 12 Recent case reports have shown better visual outcomes when systemic acyclovir and steroids are prescribed. 5 6 MacKinnon et al 2 reported a case of VZV neuroretinitis in a 9-year-old child. The patient had blurred vision of the right eye with a VA of 0.01, after an episode of varicella.…”

“…The degree of visual loss varies, ranging from mild to severe visual impairment [ 6 12 ]. Adults, especially the elderly, are more affected; however, HZO-related ON in children has also been reported [ 9 , 13 ]. HZO-related ON in many previous reports was found simultaneously with other ocular complications, such as anterior uveitis, keratitis, and secondary glaucoma.…”
