A Case of Spondylodiscitis and Paraspinal Abscess Due to Gemella haemolysans

Özet 56 yaşında erkek hasta beş aydır devam eden bel ağrısı yakınması ile başvurdu. Lumbo-sakral vertebra manyetik rezonans görüntüleme tetkikinde L5-S1 intervertebral diskinde spondilodiskit ve paraspinal kaslarda apse ile uyumlu olan yoğun kontrast madde tutulumu izlendi. Kan kültürlerinde Gemella haemolysans üredi. Mikroorganizmanın antibiyogram sonucunda amoksisilin-klavulanat ve siprofl oksasine duyarlı olduğu görüldü. Altı aya tamamlanan tedavi sonunda hastanın bel ağrısı tamamen kayboldu. G. haemolysans'ın nadir de olsa spondilodiskit etyolojisinde rol oynayabileceği unutulmamalıdır. AbstractA 56 year-old male presented to the hospital for low back pain of fi ve-months duration. On lumbo-sacral magnetic resonance imaging, a dense contrast enhancement compatible with spondylodiscitis involving L5-S1 intervertebral disk, together with a paraspinal abscess was detected. Gemella haemolysans grew in blood cultures. The isolate was susceptible to amoxicillin-clavulanate and ciprofl oxacin according to antibiotic susceptibility testing. At the end of the six-month treatment, the patient's low back pain recovered completely. It should not be forgotten that G. haemolysans may play a role in the etiology of spondylodiscitis despite its rarity. Klimik Dergisi 2010; 23(3): 138-40.
