A case report of brain abscess caused by Nocardia farcinica

Abstract: Background Brain abscess due to the Nocardia genus is rarely reported and it is usually found in immunocompromised patients. Treatment of Nocardia brain abscess is troublesome and requires consideration of the severity of the underlying systemic disease. The difficulties in identifying the bacterium and the frequent delay in initiating adequate therapy often influence the prognosis of patients. Case presentation Here, we report a rare case of brain… Show more

“…There are only 9 strains that are isolated from eyes among 441 different Nocardia strains collected by Wang et al [ 1 ] N. farcinica usually causes mild keratitis through exogenous routes (e.g., lens abrasion) or causes more severe endophthalmitis, invades the brain, lungs or other tissues, leading to purulent lesions through endogenous routes (e.g., hematogenous dissemination). [ 11 , 12 ] Once the infection occurs, the onset of disease is acute, but the development is rapid with strong invasiveness, severe tissue destruction and long course of treatment. [ 13 ] The patient in this study is a decoration worker.…”

“…Literature is limited to case reports and few case series published, although incidence seems to be growing [3]. Sixteen different species of Nocardia are known to cause clinical manifestations in humans, with the most commonly reported species in the literature being N. asteroides, N. farcinica, and N. cyriacigeorgica [1,4].…”

“…Cerebral nocardiosis is rare and usually manifests as brain abscesses, although cerebral infiltration, meningitis, ventriculitis, and spinal cord infections have been reported [3]. Nocardial brain abscess accounts for 2% of all cerebral abscesses carrying the highest mortality rate among bacterial abscesses [2,4,5].…”

“…Our case is consistent with this pattern, that is multiple cavities occurring and changing over a short period of time. The next is the nervous system, common imaging findings include brain abscess, granulomatous formation or diffuse infiltration [ 16 , 17 ], meningitis, epidural and paraspinal abscesses [ 18 , 19 ]. This case has no typical brain MRI performance.…”
