A clinical and histopathological study of histoid leprosy

Abstract: The majority of cases in this study were de novo. HL can occur even when there is no history of inadequate or irregular treatment. Papules rather than nodules were the most common skin lesions seen. Earlobe infiltration was seen in the majority of cases unlike that reported in the literature. Lepra reactions are very rare in HL.

“…2 A Study by Sukumaran Pradeep et al observed Lepra reactions in 23.52% of histoid cases. 3 Clinically lesions are shiny, firm, dome-shaped, skin-coloured, or reddish papules or nodules stretching the overlying skin with normal appearing surrounding skin. Other types of lesions are subcutaneous nodules, deeply fixed cutaneous nodules, soft nodules, and plaques or pads.…”

“…The differential diagnosis of HL must include xanthoma, neurofibroma, dermatofibroma, reticulohistiocytosis, and cutaneous metastasis [3]. Although it is difficult to diagnose HL, some clinical clues can help, such as a previous diagnosis of leprosy treated with dapsone monotherapy, abnormal neuropathy, and leonine facies [5, 6]. LL usually does not affect the sensation [7].…”

“…However, de novo cases have also been described. [ 2 ] HL usually presents with localized rather than generalized lesions, and can thus be missed clinically. The papules and nodules are usually distributed over the trunk and extremities (favoring the elbows and knees), and the face.…”
