“…Previous studies [29,30] have demonstrated significant inconsistencies between RSB estimations by various direct and indirect calculations and highlight the value of directly measuring RSB via intraoperative pachymetry measurements.…”

“…Avoiding retreatment in corneas with low residual stromal bed thickness As corneal thickness measurements taken months after initial LASIK usually overestimate RSB thickness at the time of LASIK retreatment [29,30], accurate assessment of actual RSB is critical to avoid excessive ablation of the posterior stroma. This can be avoided by utilizing intraoperative pachymetry measurements prior to laser ablation, or by utilizing confocal microscopy prior to enhancement, as the confocal microscope can accurately measure RSB thickness without ever lifting the flap [58].…”

When ectasia develops, early recognition and proper management are essential to prevent progression, to promote visual rehabilitation, and to reduce the need for corneal transplantation for these patients.

“…3,14,15 Residual stromal bed can be calculated with ultrasound pachymetry by various methods before and after LASIK. 26,27 In our study, the measured residual stromal bed at retreatment was thinner than the estimated residual stromal bed at primary LASIK in most eyes. Muallem et al 28 found a mean difference of 3 G 29 mm; the residual stromal bed thickness at enhancement was thicker than the estimated thickness after primary LASIK, a finding that is opposite to the finding in our study.…”
Intraoperative pachymetry of the stromal bed during retreatment is strongly recommended as the residual stromal bed and flap thickness changes between primary and retreatment. There is a tendency for the measured stromal bed at retreatment to be thinner than the calculated stromal bed and for the flap to be thicker than previously measured.
