A Comprehensive Assessment Protocol for Swallowing (CAPS): Paving the Way towards Computer-Aided Dysphagia Screening

Abstract: Dysphagia is one of the most common problems among older adults, which might lead to aspiration pneumonia and eventual death. It calls for a feasible, reliable, and standardized screening or assessment method to prompt rehabilitation measures and mitigate the risks of dysphagia complications. Computer-aided screening using wearable technology could be the solution to the problem but is not clinically applicable because of the heterogeneity of assessment protocols. The aim of this paper is to formulate and unif… Show more

“…In addition, only one study accounted for the non-swallowing task. A previous review commented that protocol heterogeneity might hinder the translational potential of wearable technology on swallowing assessment ( So et al, 2023 ) and that a unified framework was necessary to account for both swallowing and non-swallowing activities ( Lim et al, 2023 ).…”

“…As dysphagia or neurodegenerative problems deteriorate gradually and aspiration could occur unexpectedly ( Lim et al, 2023 ), it is necessary to develop accessible and reliable instrumental screening tools that enable continuous or frequent assessments of aspiration risks. Wearable technology with computer-aided diagnosis/screening might be a potential alternative to bedside questionnaires and instrumental diagnostic instruments (such as VFSS and FEES) ( So et al, 2023 ).…”

“…Secondly, our protocol design did not purport to cover every swallowing task. While we took reference from the comprehensive assessment protocol for swallowing (CAPS) [91], identifying the fewest swallowing tasks necessary to accurately depict swallowing functions would be helpful to develop the instrument for dysphagia screening and lessen the time and inconvenience during the assessment, which warrant further investigations. The inclination of the camera was determined based on our pilot experiment that better captured the frontal view of the neck area.…”

“…21,22 The clinical bedside swallowing examination (CBSE) is a noninstrumental assessment used to acquire essential cognitive and behavioral information on physical and functional swallowing abilities. 23 It is a sufficient aspiration screening tool, and in particular, it provides a thorough and organized evaluation of swallowing components and an assessment of the risk of dysphagia. 24 During this clinical assessment, a food bolus is given to the patient, and swallowing is examined by the speech and language pathologist (SLP).…”
