A Cost-Utility Analysis Comparing Oncoplastic Breast Surgery to Standard Lumpectomy in Large Breasted Women

Abstract: Purpose: Ablative options, beyond mastectomy, for large breasted patients with breast cancer include oncoplastic resection via reduction pattern and standard lumpectomy. Oncoplastic resection also entails a contralateral procedure for symmetry and the potential benefit of a superior cosmetic outcome. Our aim was to examine the cost-effectiveness of this treatment strategy comparing it to standard lumpectomy in treating breast cancer patients. Methods: A literature review was performed of the probabilities and … Show more

“…Overall, our analysis relates to and expands on previously published data demonstrating that patients undergoing OPS had the lowest complication rates when compared to patients undergoing M alone or M with any reconstructive procedure [20,21,23,26,27,28]. OPS is gaining popularity due to its high aesthetic satisfaction, increased tumor free margin rate, and decreased recurrence rate when compared to other surgical interventions [1,15,16,22,26,29].…”

“…Only one study on Complete Economic Evaluation (CEE) was identified during this systematic review and indicated an overall cost-effectiveness of large volume oncoplastic surgery when compared to standard lumpectomy [ 59 ]. The cost-effectiveness analysis is described in the attached document (Supplemental file 8).…”

“…Additionally, women with symptomatic macromastia undergoing oncoplastic operations that use a breast reduction design receive the benefits of symptomatic relief as well as the oncologic benefits inherent to oncoplastic surgery 26 . As such, past studies comparing the value of oncoplastic surgery to other breast cancer surgical options in women with symptomatic macromastia and breast cancer have proven its cost‐effectiveness 27–29 . Oncoplastic surgery has been shown to be a reasonable option in patients with relatively larger cancers in moderate‐to‐large breasts with associated ptosis 30,31 .…”
