Background and objectives: Oral mucositis is one of the main adverse events of cancer treatment with chemotherapy or radiation therapy. It presents as erythema, atrophy or/and ulceration of oral mucosa. It occurs in almost all patients, who receive radiation therapy of the head and neck area and from 20% to 80% of patients who receive chemotherapy. There are few clinical trials in the literature proving any kind of treatment or prevention methods to be effective. Therefore, the aim of this study is to perform systematic review of literature and examine the most effective treatment and prevention methods for chemotherapy or/and radiotherapy induced oral mucositis. Materials and methods: Clinical human trials, published from 1 January 2007 to 31 December 2017 in English, were included in this systematic review of literature. Preferred reporting items for systematic reviews and meta-analysis (PRISMA) protocol was followed while planning, providing objectives, selecting studies and analyzing data for this systematic review. “MEDLINE” and “PubMed Central” databases were used to search eligible clinical trials. Clinical trials researching medication, oral hygiene, cryotherapy or laser therapy efficiency in treatment or/and prevention of oral mucositis were included in this systematic review. Results: Results of the studies used in this systematic review of literature showed that laser therapy, cryotherapy, professional oral hygiene, antimicrobial agents, Royal jelly, L. brevis lozenges, Zync supplementation and Benzydamine are the best treatment or/and prevention methods for oral mucositis. Conclusions: Palifermin, Chlorhexidine, Smecta, Actovegin, Kangfuxin, L. brevis lozenges, Royal jelly, Zync supplement, Benzydamine, cryotherapy, laser therapy and professional oral hygiene may be used in oral mucositis treatment and prevention.

Abstract. Severe stomatitis may lead to the need to interrupt or discontinue cancer therapy and, thus, may affect control of the primary disease. Stomatitis may also increase the risk of local and systemic infection and significantly affects the quality of life and the cost of care. The present study was conducted to evaluate the efficacy of two traditional herbal medicines in controlling treatment-induced stomatitis in a small cohort of lung cancer patients treated with afatinib. All patients who were treated with afatinib for epidermal growth factor receptor (EGFR) mutated nonsmallcell lung cancer (NSCLC) between January, 2015 and March, 2016, were included in this study. During the study period, a total of 14 NSCLC patients were treated with afatinib, an EGFR-tyrosine kinase inhibitor (TKI). Two patients already had stomatitis at the time of initiation of afatinib therapy; among the remaining 12 NSCLC patients, 2 (16.7%) developed stomatitis. All the lesions in the 4 patients who developed stomatitis were completely alleviated after 2 weeks of therapy with Aznol mouthwash, a chamomile extract with anti-inflammatory effects, and Hangeshashinto, a traditional herbal (Kampo) medicine. Afatinib therapy was re-initiated, but none of the patients developed stomatitis thereafter. To the best of our knowledge, this is the first report evaluating oral care and management of stomatitis. This type of care and treatment may reduce the incidence of complications associated with EGFR-TKI therapy.

ÖZKanser tedavisine bağlı gelişen oral mukozit; ağrı, beslenmede yetersizlik, kilo kaybı ve tedavinin ertelenmesi gibi birçok soruna yol açan ve hastaların yaşam kalitesini olumsuz yönde etkileyen önemli bir sorundur. Mevcut ağız bakımı yaklaşımlarına rağmen bu sorun güncelliğini korumakta ve yeni yöntemlerin etkinliği araştırılmaktadır. Etkinliği araştırılan uygulamalar arasında tamamlayıcı tedaviler, sağlık profesyonellerinin oral mukozit yönetimine ilişkin kanıt oluşturmak amacıyla sıklıkla araştırma yaptığı alanlardan biri haline gelmiştir. Kriyoterapi, düşük yoğunluklu lazer tedavisi, bitkisel yaklaşımlar ve besin destekleri, apiterapi, esansiyel yağlar ve bilişsel davranışçı yaklaşımlar oral mukozit yönetiminde etkinliği araştırılan tamamlayıcı tedavi yaklaşım-ları arasında yer almaktadır. Bu derlemede kanser tedavisine bağlı gelişen oral mukozit yönetiminde etkinliği araştırılmış tamamlayıcı tedavi yaklaşımlarına ilişkin araştırma örnekleri sunulmuş ve bu yöntemlerle ilgili kanıta dayalı uygulama rehberlerinde yer alan önerilere yer verilmiştir.
