A four‐phase strategy for the implementation of reflectance confocal microscopy in dermatology

Abstract: This is the first study that shows a method for implementation of RCM in dermatology. The strategy and hands on protocol for psoriasis may serve as a model for other dermatological entities and additionally may lead to specialized ready-to-use RCM protocols for clinical dermatological practice.

“…This view is shared by other authors, who also highlighted the necessity to raise awareness of the existing technologies within the medical community, standardize protocols, determine clinically relevant parameters such as sensitivity and specificity in large clinical trials and determine the cost‐effectiveness of the technologies. This approach has been proven successful for RCM, for which a protocol for the diagnosis of chronic plaque psoriasis in dermatological practice has been proposed . The protocol was based on the knowledge of disease features recognizable in RCM images, gained through a series of preliminary clinical studies in psoriasis expertise centres, and on a sound knowledge of relevant literature.…”

“…This approach has been proven successful for RCM, for which a protocol for the diagnosis of chronic plaque psoriasis in dermatological practice has been proposed. [29] The protocol was based on the knowledge of disease features recognizable in RCM images, gained through a series of preliminary clinical studies in psoriasis expertise centres, and on a sound knowledge of relevant literature. As such challenges will be overcome, and more affordable technological implementations will be brought forward, chances are high that the widespread application of biophysical analysis techniques in dermatological practice and in cosmetic sciences will prove invaluable in offering personalized and noninvasive skin treatment solutions.…”

“…The fluorescence scanning confocal microscopy is typically used for imaging in the majority of biological applications [ 68 70 ]. The use of reflectance confocal microscopy for tissue imaging is limited, but sometimes it can provide additional information from the samples with significant spatial variation of refractive index [ 71 72 ]. It is worth to mention that optical sections are imaged in a focal plane tangential to the tissue surface.…”

“…Confocal microscopy has been extensively used in different branches of medicine [ 69 72 ]. Due to its ability to provide real-time structural information on superficial layers of tissue this technique was also applied for the detection of precancerous lesions of the uterine cervix [ 73 76 ].…”

“…4 Descriptive statistics were used to characterize the survey responses. As RCM's use becomes standardized and streamlined into clinical use, 5,6 there may be a need to systematically train residents in the acquisition and interpretation of its images. This national survey is one of the first to characterize RCM's current integration into residency education.…”
