A Global, Multi-Disciplinary, Multi-Sectorial Initiative to Combat Leptospirosis: Global Leptospirosis Environmental Action Network (GLEAN)

Abstract: Leptospirosis has emerged as a major public health problem in both animals and humans. The true burden of this epidemic and endemic disease is likely to be grossly under-estimated due to the non-specific clinical presentations of the disease and the difficulty of laboratory confirmation. The complexity that surrounds the transmission dynamics, particularly in epidemic situations, requires a coordinated, multi-disciplinary effort. Therefore, the Global Leptospirosis Environmental Action Network (GLEAN) was deve… Show more

Per-patient costs (USD), DALYs incurred, antibiotic overuse (Prob(over)) and underuse (Prob(under)) for febrile patients seeking care on the fourth day (average day) of illness and undergoing various test and treat strategies, with 75% antibiotic effectiveness. = strategies on the effectiveness frontier (economically efficient) for Scenario A (bacterialendemic); º = strategies on the effectiveness frontier (economically efficient) for Scenario B (viral-endemic).…”

“…Depending on the specific etiology, if untreated, patients could experience respiratory distress, multi-organ failure, and even death [2,3]. Due to climate change and the increasing counts of weather-related disasters [4][5][6], healthcare providers are facing a growing number of febrile illness patients, especially in low-resource settings [7]. The global annual burden of diseases under the umbrella of acute febrile illness is estimated to be on the order of millions of cases [8,9].…”

“…Por se tratar de uma doença bacteriana que se apresenta clinicamente de diversos modos e com uma complexidade inerente à forma de transmissão, em que mais de duas centenas de possíveis reservatórios estão inter-relacionados com uma multidão de sorovares patogênicos, por si só já constitui um desafio para o adequado diagnóstico. Aliado a isso, a dificuldade em desenvolver tecnologias de diagnóstico de menor custo, precoce e acurado colabora para a incerteza quanto à real carga global da doença 14,15,20 .…”
