A hidden culprit for ventricular dysfunction in aortopulmonary window repair: Anomalous origin of left coronary artery. Case report and review of literature

Abstract: Two-dimensional echocardiography falsely suggesting normal origin of left coronary artery. Central MessageDirect intraoperative visualization of the coronaries os is essential before APW repair. Routine clinical evaluation and imaging are unreliable in ruling out the rare association AL-CAPA with APW.

“…Association of AP Window with ALCAPA is extremely rare, and to date, we found 11 published articles with this association, all case reports, and none with preoperative diagnosis. 7,8 Multiple reports of the association of APW and coronary arteries anomalies are published, but most of them are with an anomalous RCA. In our case, the ALCAPA was diagnosed intraoperatively after removing aortic cross-clamp without reperfusion of the LV, allowing immediate correction of the defect and an adequate recovery for the patient.…”

“…We found only one patient with associated congenital heart disease in our series, who had APW. Occurrence of ALCAPA in a patient with APW is extremely rare with only a few cases described in the literature 19,20 . In almost all of these cases, ALCAPA was diagnosed intraoperatively.…”

Identification of ALCAPA in a patient with APW in our series also highlights the crucial role of preoperative CT angiography to avoid intraoperative adverse events.…”
