This paper presents an integer programming formulation for the hospital re-planning problem which arises after hospital network mergers. The model finds the best re-allocation of resources among hospitals, the assignment of patients to hospitals and the service portfolio to minimize the system costs subject to quality and capacity constraints. An application in the Turkish hospital networks case is illustrated to show the implications of consolidation of health insurance funds on resource allocations and flow of patients in the system.

Stress placed on network infrastructure by the popularity of the World Wide Web may be partially relieved by keeping multiple copies of Web documents at geographically dispersed locations. In particular, use of proxy caches and replication provide a means of storing information 'nearer to end users'. This paper concentrates on the locational aspects of Web caching giving both an overview, from an operational research point of view, of existing research and putting forward avenues for possible further research. This area of research is in its infancy and the emphasis will be on themes and trends rather than on algorithm construction. Finally, Web caching problems are briefly related to referral systems more generally.

Este trabalho tem por objetivo comparar a distribuição de unidades hospitalares, considerando os diferentes graus de especialização, em Santa Catarina, Brasil, com a distribuição de unidades resultantes da aplicação do modelo hierárquico de p-medianas, em três níveis. O modelo de p-medianas é usado para determinar a localização das unidades, e a seguir é comparado o deslocamento médio da população para alcançar as unidades médicas nos dois cenários, o atual e o simulado. Um indicador quantitativo de acessibilidade é proposto e é usado para avaliar a acessibilidade da distribuição atual com a simulada. O trabalho tem o intuito de revelar regiões subatendidas, e de servir de ferramenta de auxílio à decisão de gestores na área de saúde para possíveis intervenções no sistema, no sentido de torná-la mais homogênea e mais acessível à população
