A Home Diary to Assess Functional Status in Patients with Parkinson's Disease with Motor Fluctuations and Dyskinesia

Abstract: In clinical trials for patients with Parkinson's disease (PD) with motor fluctuations, efficacy is generally ascribed to an intervention if motor function is significantly improved or if "off" time is significantly reduced. However, we have argued that patients might not be improved if off time is reduced only to the extent that unwanted dyskinesia is increased. Therefore, a home diary should include an assessment of dyskinesia to provide an accurate reflection of clinical status over a period of time. We unde… Show more

“…Motor state diaries were obtained at baseline, at 3 months, and 6 months postoperatively. 9 Subjects were trained on how to complete motor diaries by scoring their motor status at half-hour intervals; the possible choices were "asleep," "off," "on," "on with nontroublesome dyskinesias," and "on with troublesome dyskinesias." 9 Subjects completed motor diaries for 3 consecutive days before the baseline, 3-month, and 6-month evaluations.…”

“…Patients also completed a 2-day motor "on-off " diary at home to estimate average daily time spent in "off " state. 12 The UPDRS part II, item 13 on falling (unrelated to freezing) was used to determine fall status, with nonfallers having a score of 0 and fallers a score of 1 or higher.…”

“…Very little research in this area has been undertaken. However, there is some evidence that keeping diaries to document the occurrence of symptoms in relation to timing of antiparkinsonian medication might increase adherence (Hauser et al 2000) and the approach is acceptable to patients (Hauser et al 2006).…”
