A method for defining the CORE of a psychosocial intervention to guide adaptation in practice: Reciprocal imitation teaching as a case example

Abstract: Defining the central components of an intervention is critical for balancing fidelity with flexible implementation in both research settings and community practice. Implementation scientists distinguish an intervention’s essential components (thought to cause clinical change) and adaptable periphery (recommended, but not necessary). While implementing core components with fidelity may be essential for effectiveness, requiring fidelity to the adaptable periphery may stifle innovation critical for personalizing … Show more

“…This finding highlights the need to specify the core elements and acceptable adaptations of structured interventions prior to implementation efforts (Chambers & Norton, 2016). Attention to this matter, may support providers in making adaptations that are responsive to diverse families and children, but also maintain the integrity of structured, parent-mediated interventions (Edmunds et al, 2022).…”

“…Research must therefore extend from “effectiveness” to theorising how and why interventions work ( 9 ). A set of key elements should be identified, as insistence on continuing fidelity to multiple, incorrect, or ineffective components can obstruct implementation and innovation ( 10 ).…”

“…delivery setting; Stirman et al, 2019). These frameworks can also be used to categorize adaptations as being either consistent or inconsistent with the core components of an intervention (Kirk et al, 2020), thus providing guidance on whether an adaptation may enhance or attenuate service outcomes (Edmunds et al, 2022; Miller et al, 2020). Within the autism field, these frameworks have been used at the conclusion of intervention delivery to categorize adaptations made to the entirety of an intervention (Dickson et al, 2021; Dyson et al, 2019).…”
