A multidimensional approach to assessing intervention fidelity in a process evaluation of audit and feedback interventions to reduce unnecessary blood transfusions: a study protocol

Abstract: BackgroundIn England, NHS Blood and Transplant conducts national audits of transfusion and provides feedback to hospitals to promote evidence-based practice. Audits demonstrate 20% of transfusions fall outside guidelines. The AFFINITIE programme (Development & Evaluation of Audit and Feedback INterventions to Increase evidence-based Transfusion practIcE) involves two linked, 2×2 factorial, cluster-randomised trials, each evaluating two theoretically-enhanced audit and feedback interventions to reduce unnecessa… Show more

“…In keeping with recommendations for triangulation [ 38 , 39 ], findings regarding barriers and facilitators of logging in and using the SAR and regarding primary and secondary trial outcomes will be summarized in 1 table, known as a convergence coding matrix. Convergence of findings across all data sets will be assessed: full agreement (data convergence), partial agreement (complementarity between data), conflicting findings (discord) or silence (finding identified in only 1 data source and no additional sources) [ 40 ]. We will examine points of divergence to better understand the relationship between theory and outcomes, find out how an intervention may work or not in particular conditions, and identify areas of opportunity for future research on how interventions could be improved.…”

“…This is currently a topic of great interest among KT researchers though there are no reports of evaluating implementation delity at a region or population level. 21 It may be of interest to integrate intervention implementation delity into the next iteration of our KT Signature Assessment Tool.…”

“…This is currently a topic of great interest among KT researchers though there are no reports of evaluating implementation fidelity at a region or population level. 21 It may be of interest to integrate intervention implementation fidelity into the next iteration of our KT Signature Assessment Tool. Finally, scores for implementation of KT interventions at a region-level may not reflect impact on patient care, or the effectiveness of said interventions.…”
