Objectives Monocyte chemoattractant protein (MCP)-1 and macrophage inflammatory protein (MIP)-loc exhibit chemotactic activity toward macrophages/monocytes and induce the production of inflammatory cytokines affecting granuloma formation. Recently, a single nucleotide polymorphism (SNP) in the MCP-1 distal regulatory region and a dinucleotide repeat in the MIP-1Agene promoter region were identified. Weinvestigated the relationships between the polymorphisms and susceptibility to sarcoidosis, clinical manifestations, and BALFfindings of sarcoidosis. Methods The polymorphisms of the MCP-1and MIP-1A genes in 118 patients with sarcoidosis and 145 healthy control subjects were examined. The MCP-1 polymorphism was genotyped by a PCR-restriction fragment length polymorphism method and the MIP-1Agenotype was determined using PCR. Results No significant difference in the genotype distribution or in the allele frequency between the patients and control subjects was observed. Wefound no relationship between the polymorphisms and the serum ACElevel, organ involvement, roentgenographic stages, or deterioration in chest radiographs during the follow-up. A significant difference in the absolute counts ofAMsin BALFof 51 patients amongthe genotypes of the MCP-1gene was found (p=0.048). The AMcounts in BALF of the G/A and G/G genotypes were significantly increased compared with that of the A/A genotype (p<0.05). Conclusion The polymorphisms of the MCP-1and MIP1Agenes do not play a substantial role in genetic predisposition for sarcoidosis or in clinical manifestations of sarcoidosis in this Japanese population. The MCP-1 SNP might be related to the recruitment of monocytes/macrophages to the alveolar spaces in sarcoidosis. (Internal Medicine 41: 813-818, 2002)

We present a large case-control candidate gene study of leprosy susceptibility. Thirty-eight polymorphic sites from 13 genes were investigated for their role in susceptibility to leprosy by comparing 270 cases with 452 controls in Karonga district, northern Malawi. Homozygotes for a silent T-->C change in codon 352 of the vitamin D receptor gene appeared to be at high risk (odds ratio [OR] = 4.3, 95% confidence interval [CI] = 1.6-11.4, P = 0.004), while homozygotes for the McCoy b blood group defining variant K1590E in exon 29 of the complement receptor 1 (formerly CD35) gene appeared to be protected (OR = 0.3, 95% CI = 0.1-0.8, P = 0.02). Borderline evidence for association with leprosy susceptibility was found for seven polymorphic sites in an additional six genes. Some of these apparent associations may be false-positive results from multiple comparisons, and several associations suggested by studies in other populations were not replicated here. These data provide evidence of inter-population heterogeneity in leprosy susceptibility.

The effect of host genetic variation on the outcome of hepatitis C virus (HCV) infection and its treatment is poorly understood. The chemokine receptors CCR5, CCR2, and CCR3 and their ligands, RANTES, MCP-1, MCP-2, and MIP-1␣, are involved in the immune responses and the selective recruitment of lymphocytes to the liver in HCV infection. We studied 20 polymorphisms within these genes and investigated their association with persistent carriage of HCV, severity of liver disease, hepatic inflammation, and response to treatment in a large European cohort. Significant associations were found between CCR5-⌬32 and reduced portal inflammation (P ‫؍‬ H epatitis C virus (HCV) is a major global health problem with 1% to 2% of the world's population chronically infected. The prevalence of infection in the United Kingdom is approximately 0.5%, 1 but it is likely that this figure will continue to rise as more cases come to light. Persistent HCV infection has a very variable outcome, with 25% of those infected developing cirrhosis within 20 years, of whom 3% to 5% per year will proceed to hepatocellular carcinoma. A further 5% per year will develop liver failure, and 2% per year will die of a liver-related death. There is also a very variable response to treatment because only 40% of those treated with interferon alfa and ribavirin successfully clear the virus. 2,3 There are a number of epidemiologic and viral factors that influence susceptibility to persistent HCV infection, progression of HCV-related liver disease, and response to treatment, but host genetic factors are also influential. This study has examined 20 polymorphisms in the genes for the CC chemokine receptors (CCR) CCR5, CCR2, and CCR3 and their ligands, RANTES (regulated and normal T-cell expressed and secreted), MCP-1 (monocyte chemotactic protein), MCP-2, and MIP-1␣ (macrophage inflammatory protein), to try and elucidate their role in the immunopathologic outcome of HCV infection..Chemokines constitute a large family of small (8-10 kD) cytokines that are up-regulated in inflammation and whose effects are mediated by members of a family of 7 transmembrane domain, G protein-coupled receptors. 4 Their major role is in leukocyte migration and dependent processes such as immune surveillance and innate and adaptive immune responses.
