ABSTRACThydrocephalus following cranial surgery is a well-recognized phenomenon (29) and results from a combination of collateral damage incurred during the surgical procedure (19) and the alterations occurring later in the CSF circulation, cerebral blood flow (CBF) auto-regulation and cerebral compliance (47).Complications resulting from VP shunts are notoriously common (7,9,13,24) and have been a subject of extensive research. (26, 27, 51) However, only a few studies have focused on VP shunt survival in patients who develop hydrocephalus as a consequence of cranial surgery itself. In this study, we report our experience of managing post-cranial surgery hydrocephalus in both pediatric and adult patients with particular emphasis on shunt complications and factors influencing shunt survival. █ INTRODUCTIONHydrocephalus results from abnormal production, flow or absorption of cerebrospinal fluid (CSF) causing an "active distension" of ventricles of the brain (40). Left untreated, this condition can lead to rapid deterioration and adverse long-term outcomes, both in pediatric (45) and adult (28) patients. Prompt CSF diversion with ventriculoperitoneal (VP) shunt insertion remains the most popular treatment for hydrocephalus (17).A number of classification schemes have been proposed to categorize hydrocephalus (3,22,32,33,37). However, no single scheme is comprehensive enough to encompass all aspects of this diverse entity (40). The development of AIM: Ventriculoperitoneal (VP) shunt insertion remains the most popular method for the treatment of hydrocephalus despite its associated complications. We assessed VP shunt survival in a group of patients who had developed hydrocephalus following cranial surgery. MATERIAL and METHODS:A retrospective charts review was done over a 10-year period at our institution. Kaplan-Meier survival curves and Log-Rank (Cox-Mantel) test were used to analyze various factors affecting VP shunt survival. RESULTS:Among the 67 cases included, a total of 28 (46.3%) patients had undergone cranial surgery for brain tumors. The overall rate of shunt failure was 14.9% at a mean follow-up of 16 months. Shunt failure in pediatric patients (20%) was slightly higher than that in adult patients (13.5%). The median time to first shunt failure was adversely influenced by a history of brain tumor (p = 0.019), prolonged antibiotic therapy (p = 0.018) and administration of steroids (p = 0.004).CONCLUSION: Shunt survival was worse in patients who developed hydrocephalus following cranial surgery performed for brain tumors and those who received either steroids or prolonged antibiotic therapy. Thus post-cranial surgery hydrocephalus represents a unique subset of hydrocephalus.

This study was designed to investigate (i) extracellular matrix to specify adhesive substrates to human dura mater cell (hDMC); (ii) the alteration on adhesion‐related molecules in hDMC; and (iii) secreted matrix metalloproteinases (MMPs) linked with extracellular matrix remodeling after exposure to inflammation. The hDMC was cultured from human dura mater tissue, and the studies were performed with hDMC after co‐culturing with macrophage like THP‐1 cells (Mϕ). The adhesion of co‐cultured hDMC through collagen I increased 6.4‐fold and through collagen IV increased 5.0‐fold compared with the adhesion of naïve cells (p < 0.001). Integrin subtype α2β1 expression was increased 6.3‐fold (p < 0.001) and α1 expression was decreased 2.0‐fold (p < 0.001) in the co‐cultured cells compared with the naïve cells. Co‐culturing induced significant increases in MMP‐1 (13.9‐fold, p < 0.01), MMP‐3 (7.6‐fold, p < 0.01), and VEGF (VEGF: 3.8‐fold, p < 0.05) expression and decreases in MMP‐9 (0.1‐fold, p < 0.01) compared with the sum of naïve hDMC and Mϕ values. Increased hDMC adhesion under inflammatory conditions is caused by an increased cellular affinity for collagen I and IV mediated by increased hDMC levels of integrin subtype α2β1 and environmental MMP‐1, ‐3 and decreased MMP‐9. Selective integrin subtype α2β1 inhibition assay showed 37.8% and 35.7% reduction in adhesion of co‐cultured hDMC to collagen I (p < 0.001) and IV (p = 0.057), respectively. The present study provides insight into the pathological conditions related to dura mater adhesion in inflammation. © 2018 Orthopaedic Research Society. Published by Wiley Periodicals, Inc. Background Regenerated oxidized cellulose (ROC) sheets have gained popularity as an adjunct to a vascularized nasoseptal flap for closure of dural defects after endoscopic endonasal skull‐base approaches (EESBS). However, evidence supporting its impact on the healing process is uncertain. This study was performed to evaluate the impact of ROC on the nasal mucosa and assess its effects on tissue pH, structure, and cell viability. Methods In 5 patients, a 1‐cm2 piece of ROC gauze was placed on the surface of the middle turbinate before it was resected as part of a standard EESBS. Mucosa treated with ROC was separated from untreated mucosa and a histologic examination of structural changes in the respiratory epithelium was performed. To assess the effect of ROC on pH, increasing amounts of ROC were added to culture medium. Nasal fibroblasts viability was assessed in the presence of ROC before and after the pH was neutralized. Results Compared with unexposed controls, treated mucosa exhibited a higher incidence of cell necrosis and epithelial cell detachment. When added to Dulbecco's modified Eagle medium, ROC caused a dose‐dependent decrease in pH of the medium. Only 1 ± 0.8% of cultured fibroblasts exposed to the ROC‐induced acidic medium were alive, whereas 98.25 ± 0.5% of the cells were viable when the pH was neutralized (p < 0.001). Conclusion ROC applied in vivo to nasal mucosa induced epithelial necrosis likely by diminishing the medium pH, because pH neutralization prevents its effect. The ultimate effect of this material on the healing process is yet to be determined.
