A New Technique for Curvilinear Scalp Reduction

Abstract: A technique of alopecia reduction utilizing a curvilinear ellipse employing a sequential excision and suture technique is described. Advantages of the technique over previously described procedures are presented. Measurements of individual scalp elasticity and alternating suture tension vectors help to produce an excellent cosmetic result with minimal complications.

“…Many refinements of scalp reduction surgery have been made since 1975. The list is quite long and includes the paramedian reduction, 10,17 mercedes reduction, 20 (S, J, and C type lateral patterns 22,26 ), U‐pattem, minireductions, 21,26 M‐plasties, 5 Frechet flap, 17 Brandy flap, 12 Unger's modified major scalp reduction, 16,26 Juri flap, 18 curvilinear reduction, 19 triple rhomboid flap, 21 and combinations of the above. No single procedure can be ideal for all patients; each patient must be considered individually.…”

“…The addition of scalp reductions has become an integral part of hair transplantation surgery for alopecia. Since the advent of scalp reduction surgery in the mid 1970s many modifications and refinements have been made in an attempt to maximize the effectiveness and final aesthetic result 1–22 . While modern scalp reduction surgery is predictable and successful, one consistent undesired sequela known as the “slot,” or “trough” frequently results 1 .…”

“…Although that procedure still remains very popular because of its simplicity, several improvements have been developed over the years. These include the Y‐plasty and its modifications, 2 the M‐plasty modification, 3 paramedian scalp reduction, 4 the Marzola lateral lift, 5 the scalp‐lift, 6 and the curvilinear reduction, 7 to name a few.…”
