Demand for Crude Palm Oil (CPO) at PT. Wijaya Borneo Tiganna for this year has increased. However, the increase in production was not in line with human error of 74% in the Fresh Fruit Bunches (FFB) Sorting Department from the 80% minimum percentage desired by the company. The purpose of this study was to analyze the reliability value of sorting workers. The method used is the Human Reliability Assessment Method seen from related tasks and context related to identify Human Error Probability (HEP) with a fault tree to provide recommendations for reducing human error. There are 3 types of rework in the TBS Sorting Section. The HEP using the HEART method is 1.1103148 while the CREAM method is 0.10608. The reliability of workers in carrying out tasks can still be improved with skills and accuracy in the sorting and grading of FFB.

The comprehensive review of research literature in accordance with the implementations of the accident causation models enables knowledge incomes for the contributing authors towards the advancement of the accident causation methodologies for administering the accidents' causal factors. For obtaining this objective, the systematic literature review meta‐analysis are developed by evaluating the parameter indicator linked to the most utilized causation methods in addressing the contributing factors in process industries. The methodological review process called preferred reporting items for systematic reviews and meta‐analyses (PRISMA) is implemented in this paper due to its capability to integrate both systematic reviews and meta‐analysis. This systematic literature review deeply investigates 149 papers by a thorough reading from the filtered 387 manuscripts published in the academic SCOPUS databases during 1980–2020. Additionally, the statistical analysis t test is performed to identify the homogeneity of literature papers resulting from research conclusions. The suggested future improvement in the domain of accident causation models is the advancement of administering the accident causal factors' values of correlations and interdependencies. While particularly for quantified accident causation models, the adoption of uncertainty analysis needs to further developed as a research suggestion in the future.

The Cognitive Reliability and Error Analysis Method (CREAM) represents one of the second‐generation approaches to human reliability assessment, taking into account the influence of environmental conditions on human error probability (HEP). In the context of CREAM, the Common Performance Conditions (CPCs) influence error probabilities. Since not all CPCs have equal impacts, this study employs the Bayesian Best Worst Method (BWM), a novel approach in group decision‐making, to assign weights to these factors. Subsequently, two techniques based on basic CREAM are proposed. The current control mode is determined in the first technique according to the experts' opinions. Then the probability of human error is calculated based on the amount of control. It is possible to provide solutions for improving control mode, based on obtained results. Therefore, in this study, the second method has been used to make suggestions to enhance human reliability. For this purpose, in the second technique, an optimization problem is formulated to select the best applicable programs for managers to enhance human reliability. The proposed bi‐objective model tries to increase the reliability of human resources by reducing human error and costs. The proposed bi‐objective model seeks to bolster the reliability of human resources by concurrently minimizing HEP and associated costs. The efficiency of the presented methods is verified through a case study in the control room of the cement factory. The results of the first technique reveal an opportunistic control mode with a corresponding HEP of 0.0198. On the other hand, the outcomes of our proposed model underscore the greater impact of improving CPC levels in reducing the probability of human error. Ultimately, the practical programs derived from our mathematical model provide decision‐makers with valuable insights to reduce the probability of human error to a mere 0.000172 through the transition from opportunistic to strategic control.
