A novel intubation technique in bilateral cleft palate pediatric patients: hard gum shield–aided intubation

Abstract: Hard gum shield-aided intubation facilitated intubation more than left paraglossal in bilateral cleft palate children with shorter intubation time, better glottic view, easier intubation, less need for laryngeal manipulation than left paraglossal intubation with no difference in complications.

“…[ 10 ] The technique was evaluated in a previous study using a gum elastic shield in patients with bilateral cleft palate to make a bridge over the defect. [ 6 ] However, the material of the shield was not clear enough in the study neither was it obvious whether the shield was uniform in size for all patients nor patient-customised. Furthermore, maintenance of face mask ventilation was not discussed in that study.…”

“…Based on a previous study,[ 6 ] a sample size of 90 was calculated to detect a difference of 30% in the CL grade of laryngoscopic view between the two groups with a two-sided Chi-square test, Type II error of 0.2% and Type I error of 0.05% using the MedCalc Statistical Software version 16.4.3 (MedCalc Software bvba, Ostend, Belgium; https://www.medcalc.org ; 2016). Data were analysed using IBM SPSS Statistics for Windows, Version 20.0.…”

“…1,2 Cleft palate is among the conditions that can cause difficult airway management, due to inability to optimally position the head, and to maldevelopment, which makes it difficult to obtain a good view of the upper airway structures. 3,4 We describe a syndromic infant with alobar holoprosencephaly, whose long-term airway patency was maintained with a modified infant pacifier.…”

“…S ir —We read the excellent article, ‘A novel intubation technique in bilateral cleft palate pediatric patients: hard gum shield‐aided intubation’, by Mahmoud et al . with interest. We are pleased that our study on left molar approach to laryngoscopy for difficult intubation was cited with the reference number ten .…”

“…We want to draw attention to two points about the article by Mahmoud et al . . Firstly, in the section of ‘Introduction’, the author mentioned that ‘Difficult airway management in uncorrected cleft palate patients has been studied by many authors (1–16)’.…”
