A Novel Mutation in Helical Domain 2 of NOD2 in Sporadic Blau Syndrome

Abstract: We report a 12-year-old girl who presented with bilateral granulomatous anterior uveitis accompanied by boggy arthritis of knee and ankle joints, intermittent fever, and nodular skin rash. She was diagnosed with sporadic Blau syndrome (early-onset sarcoidosis) based on above clinical signs and presence of non-necrotising granuloma on iris biopsy. DNA sequencing revealed a previously unreported heterozygous mutation consisting of a G>A transition in exon 4 of the NOD2 gene. This resulted in a glutamic acid to l… Show more

“…However, dermatological assessment and granulomatous histology rectified the diagnosis to granulomatous panniculitis. Subcutaneous nodules have also been described in the literature . The term EN, which is often used in the BS literature to describe these lesions, corresponds histologically to septal panniculitis which, per definition, must not be granulomatous.…”

“…Though early onset sarcoidosis has been reported from India, to the best of our knowledge this is the first case series of molecularly confirmed familial BS from India. [67]…”

“…[ 2 ] Till date, 13 NOD2 gene mutations causing BS have been described. [ 3 4 ] Those not associated with any known gene mutations have been termed sporadic BS/early-onset sarcoidosis. [ 5 ] Uveitis in BS is in the form of progressive panuveitis with multifocal choroiditis.…”
