Abstract. Increasing age is a risk factor for the development of colorectal adenomas and advanced adenomas. However, few studies have been published on the features of colorectal polyps in the elderly. The present study aimed to investigate the clinical, enteroscopic and pathological characteristics of colorectal polyps in Chinese elderly patients in a single center (The Central Hospital of Wuhan, Hubei, China). The endoscopic and pathological reports of colonoscopies performed in our center were retrospectively analyzed. A total of 7,795 consecutive patients referred for colonoscopy were evaluated between January 2013 and December 2014. Of the 297 who met the inclusion criteria, 279 polyps were observed in men and 230 in women. Of all the polyps, 263 were non-adenomatous polyps, 104 were non-advanced adenomas and 142 were advanced adenomas. 336 polyps were left-sided and 173 were right-sided. Polyps ≥10 mm were more likely to exhibit an adenomatous component and advanced features, and these findings continued to hold true when the size cut-off was set at 5 mm. The data shown in the present study have revealed that a significant number of polyps lie proximal to the splenic flexure. Thus, evaluation of the whole bowel is particularly important in elderly patients who are undergoing colonoscopy. In addition, the polyp size was associated with the presence of adenoma, and advanced component, diminutive and small polyps should not be ignored in elderly patients.

ÖZAmaç: Kolorektal hastalıklarının tanı ve tedavisi için kolonoskopi işlemi öncesi barsak hazırlığının yeterli olması çok önemlidir. Biz bu çalışmada; kolonoskopi işlemi ön-cesinde oral ve lavman yoluyla sağlanan kolon temizliği-nin etkinliğini ve hastaların uyumunu değerlendirilmeyi amaçladık.Yöntemler: Çalışmaya kolonoskopi planlanan ve 250 ml Sennozit A-B ve kalsiyum tuzunu içeren solüsyon ve 210 ml sodyum hidrojen fosfat içerikli lavman ile barsak hazırlığı yapılan 400 hasta çalışmaya alındı. Kolonoskopi ön-cesinde kolonoskopi işlemine hazırlık ve işlem esnasında yaşamış olabilecekleri soruları içeren anket uygulandı. Kolonoskopi işlemi öncesinde hastaların kendilerinden demografik, klinik ve barsak hazırlığına yönelik 14 sorudan oluşan anketin doldurulması istendi. Anketin sonuç-ları değerlendirildi. 6±27.5cm (142-197), 76.8±9.7kg (47-152), 27.9±4.2 kg/m² (22-41) olarak bulundu. Kolon Anketi soruları ve sonuçlarından hastaların oral, rektal temizliğe ve diyete uyumlulukları sırasıyla %93.2, %90.5, ve %88.0 olarak bulundu. İşlemden bir gün öncesinde günlük alınan sıvı miktarı ise büyük bir çoğun-lukta 3litre ve üzeri olarak bulundu (%71.8). Hastaların kolon temizliği sırasında karşılaştıkları şikayet ve bulguların başında bulantı (%30.3), karın ağrısı ya da kramp (%15.5) ve şişkinlik (%12.8) gelmekteydi. Hastaların yaklaşık %45.5'i işlemin başarısız olması durumunda kolon temizliğinin zor olmasından ve de işlemin kendi zorluğu nedeniyle tekrar yaptırmayacağını bildirdi. Hastaların ço-ğunluğunun anestezi Skoru ASA I ve kolon temizliği iyi olarak değerlendirildi. BulgularSonuç: Kolon temizliği için, Sennozit A-B ve kalsiyum tuzunu içeren oral solüsyon ve 210 ml sodyum dihidrojen fosfat ve disodyum hidrojen fosfat içerikli rektal lavman güvenle kullanılabilir.Anahtar Kelimeler: Kolonoskopi, kolon hazırlığı, anket. Kolorektal hastalıklarının tanı ve tedavisinde önemli yer tutan kolonoskopi işleminin başarısı için kolonoskopi öncesi barsak hazırlığının yeterli olması gereklidir. Hastanede yatan hastalarda barsak hazırlığı ayaktan hastalara göre daha yetersiz olup bunun nedenleri belirlenmiş değildir. Bu çalışmada; standart barsak hazırlığı uygulanan yatan hastalarda barsak hazırlığının yetersizliğine neden olan faktörleri araştırmayı amaçladık. Gereç ve Yöntemler: Çalışmaya kolonoskopi planlanan ve barsak hazırlığı standart sıvı sodyum fosfat ile yapılan ardışık 66 hasta alındı. Kolonoskopi öncesi hastalara demografik, klinik ve barsak hazırlığına ilişkin verileri ile ilişkili anket yapıldı. İşlem sonrasında hastanın barsak hazırlığı yeterliliği için kolonoskopiyi yapan hekime yönelik sorularla değerlendirme yapıldı. Bulgular: 66 hastanın 23'ü yatan, 43'ü ayaktan tedavi gören hasta idi. Gruplar arasında hazırlık ilaçlarına uyum benzer iken ayaktan hastalarda berrak sıvı diyete uyum daha iyi idi (p<0,01). Grupların hazırlık ilaçlarına tolerans düzeyleri anlamlı farklılık göstermedi (P>0,05). Yatan hastalarda yeterli barsak hazırlığı ayaktan hastalara göre daha düşüktü ve sadece 6 yatan hastada yetersiz barsak hazırlığı saptandı. Yetersiz barsak hazırlığı ile berrak diyete uyum arasında negatif yönde bir ilişkili saptandı (rho= -0.351 p=0.004). Sonuç: Yatan hastalarda ayaktan hastalara göre barsak hazırlığı yetersizdir. Bu hasta grubunda yetersiz barsak hazırlığı nedenlerinden biri berrak diyete uyumsuzluk olarak gözükmektedir. Bu nedenle, yatan hastanın berrak diyete uyumunu artırıcı yöntemler geliştirilmelidir.
