A policy for the disposal of autologous hematopoietic progenitor cells: report from an Italian consensus panel

Abstract: A formal consensus process allowed SIDEM and GITMO to propose a policy for autologous HPC disposal that fulfills clinical, ethical, and economic criteria.

“…Given the frequency of collecting autologous hematopoietic progenitor cells (AHPCs) for clinical use and storage, it would be desirable to have a systematic means of making determinations regarding their appropriate disposition. Accordingly, an Italian expert panel used a structured method, achieved consensus, and offered recommendations on point that appear in this edition of TRANSFUSION . While these recommendations seem to offer a reasonable starting point to address many practical and ethical issues regarding the storage and disposal of AHPCs from a clinical perspective, it is worth considering how they might be refined over time.…”

“…In developing its recommendations for the “appropriate disposal” of AHPCs the consensus panel tried to strike a “balance between avoided storage costs and missed unplanned reinfusion opportunity.” Through the consensus process three clinical “values” surfaced that center on clinical use and one on the costs associated with storing AHPCs. Ultimately, in the recommendations proffered, clinical criteria properly trump premature disposal of AHPCs.…”
