BackgroundThe positive deviance approach focuses on those who demonstrate exceptional performance, despite facing the same constraints as others. ‘Positive deviants’ are identified and hypotheses about how they succeed are generated. These hypotheses are tested and then disseminated within the wider community. The positive deviance approach is being increasingly applied within healthcare organisations, although limited guidance exists and different methods, of varying quality, are used. This paper systematically reviews healthcare applications of the positive deviance approach to explore how positive deviance is defined, the quality of existing applications and the methods used within them, including the extent to which staff and patients are involved.MethodsPeer-reviewed articles, published prior to September 2014, reporting empirical research on the use of the positive deviance approach within healthcare, were identified from seven electronic databases. A previously defined four-stage process for positive deviance in healthcare was used as the basis for data extraction. Quality assessments were conducted using a validated tool, and a narrative synthesis approach was followed.Results37 of 818 articles met the inclusion criteria. The positive deviance approach was most frequently applied within North America, in secondary care, and to address healthcare-associated infections. Research predominantly identified positive deviants and generated hypotheses about how they succeeded. The approach and processes followed were poorly defined. Research quality was low, articles lacked detail and comparison groups were rarely included. Applications of positive deviance typically lacked staff and/or patient involvement, and the methods used often required extensive resources.ConclusionFurther research is required to develop high quality yet practical methods which involve staff and patients in all stages of the positive deviance approach. The efficacy and efficiency of positive deviance must be assessed and compared with other quality improvement approaches.PROSPERO registration numberCRD42014009365.

Purpose:As multi-institutional research networks assume a central role in clinical research, they must address the challenge of sustainability. Despite its importance, the concept of network sustainability has received little attention in the literature, and the sustainability strategies of durable scientific networks have not been described.Innovation:The Health Maintenance Organization Research Network (HMORN) is a consortium of 18 research departments in integrated health care delivery systems with over 15 million members in the United States and Israel. The HMORN has coordinated federally funded scientific networks and studies since 1994. This case study describes the HMORN approach to sustainability, proposes an operational definition of network sustainability, and identifies 10 essential elements that can enhance sustainability.Credibility:The sustainability framework proposed here is drawn from prior publications on organizational issues by HMORN investigators and from the experience of recent HMORN leaders and senior staff.Conclusion and Discussion:Network sustainability can be defined as (1) the development and enhancement of shared research assets to facilitate a sequence of research studies in a specific content area or multiple areas, and (2) a community of researchers and other stakeholders who reuse and develop those assets. Essential elements needed to develop the shared assets of a network include: network governance; trustworthy data and processes for sharing data; shared knowledge about research tools; administrative efficiency; physical infrastructure; and infrastructure funding. The community of researchers within a network is enhanced by: a clearly defined mission, vision and values; protection of human subjects; a culture of collaboration; and strong relationships with host organizations. While the importance of these elements varies based on the membership and goals of a network, this framework for sustainability can enhance strategic planning within the network and can guide relationships with external stakeholders.

Background: Knowledge management is an attempt to discover the hidden treasure of knowledge in the minds of people and turn this treasure to organizational assets so that people who are involved in the decision making of the organization can use it. Besides, the application of knowledge management, as an organizational improvement technique, is required to assess the quality of working life (QWL). Objectives: This study aimed to determine the association between knowledge management and dimensions of QWL in nurses working in Zahedan teaching hospitals. Materials and Methods:In this cross-sectional study, all nursing personnel of six teaching hospitals in Zahedan, 2014, were selected using the stratified random sampling. Data were collected using two standardized Likert-style questionnaires. Data were then entered into SPSS 17.0 software and analyzed using Pearson, t test and the one-way ANOVA test. Results: Knowledge management score in single and younger participants was higher compared to other participants and it was lower in the larger hospitals. Quality of working life score in the smaller hospitals was higher than the larger hospitals (P = 0.002). There was a strong correlation between knowledge management and all dimensions of QWL in teaching hospitals (P = 0.001). Conclusions: Knowledge management can improve the QWL in nursing personnel in Zahedan teaching hospitals. Based on the findings of the study, hospital managers, especially head nurses, could play a crucial role in improving QWL by institutionalization of knowledge management in nursing personnel and sharing and developing knowledge to advance the practice.
