A Propensity Score Analysis of the Impact of Dexamethasone Use on Delayed Cerebral Ischemia and Poor Functional Outcomes After Subarachnoid Hemorrhage

The study aimed to identify key genes involved in cerebral vasospasm (CVS) after subarachnoid haemorrhage (SAH). GSE46696 of basilar arteries of SAH mice and normal controls were downloaded from the Gene Expression Omnibus (GEO). Integrated microarray analysis was performed to identify differentially expressed genes (DEGs). GO and KEGG pathway enrichment analyses of DEGs were performed with ClueGO. The protein-protein interaction (PPI) networks were constructed using Cytoscape software. A total of 4103 DEGs were identified; among them, 254 DEGs (63 up-regulated genes and 191 down-regulated genes) showed significant differences at p < 0.05. GO analysis showed that the identified DEGs were over-represented in 16 GO terms. KEGG pathway analysis showed that pathways in immune inflammation were significantly enriched pathways for DEGs. Efektyviausi plyšusių intrakranijinių aneurizmų gydymo būdai yra chirurginis klipsavimas ir endovaskulinis kodavimas. Taikant endovaskulinį gydymą, ankstyvų mirčių ir komplikacijų tikimybė yra mažesnė, ypač užpakalinės cirkuliacijos aneurizmų atveju, tačiau išlieka didelė aneurizmos rekanalizacijos ir plyšimo rizika, todėl indikuotinas ilgalaikis stebėjimas. Tuo tarpu klipsavimo ilgalaikė nauda yra labai svarbi jaunesnio amžiaus pacientams ir tam tikrais atvejais yra pirmo pasirinkimo gydymo metodas. Aneurizmos intervencinį gydymą rekomenduojama atlikti kiek galima anksčiau. Siekiant išvengti komplikacijų, visiems pacientams, patyrusiems spontaninę subarachnoidinę hemoragiją (SAH), rekomenduojama kasdien ar kas antrą dieną atlikti transkranijinę doplerografiją, o galvos kompiuterinę tomografiją, kompiuterinės tomografijos angiografiją (KTA), kompiuterinės tomografijos perfuziją ar skaitmeninę subtrakcinę angiografiją – hospitalizavimo dieną, 3-5 dieną ir 7-10 dieną. Vėlyvos smegenų išemijos prevencijai rekomenduojama skirti nimodipiną ir palaikyti euvolemiją, skiriant izotoninius kristaloidinius tirpalus. Įvykus vazospazmui, rekomenduojama palaikyti euvoleminę indukuotą hipertenziją ir tam tikrais atvejais taikyti endovaskulinį gydymą intraarteriniais vazodilatatoriais ar (ir) angioplastiką. Neplyšusių intrakranijinių aneurizmų gydymo taktikos pasirinkimas priklauso nuo natūralios ligos eigos ir plyšimo rizikos, kurią galima įvertinti naudojantis įvairiomis skalėmis. Parinkus stebėjimo taktiką, siūloma kartoti KTA ar magnetinio rezonanso tomografijos angiografiją (MRA) po 6-12 mėnesių nuo aneurizmos nustatymo ir vertinti pokyčius dinamikoje. KTA ir MRA taip pat rekomenduojamos asmenims, turintiems 2 ar daugiau šeimos narių, kuriems nustatyta neplyšusių intrakranijinių aneurizmų ar diagnozuota SAH bei esant kitiems rizikos veiksniams. Pacientams, kuriems atsitiktinai aptikta besimptomė neplyšusi aneurizma, rekomenduojama mesti rūkyti, periodiškai matuoti kraujospūdį ir vengti sunkių svorių kėlimo.

Neuroinflammation as a Target for Intervention in Subarachnoid Hemorrhage

Aneurysmal subarachnoid hemorrhage (SAH) is a sub-type of hemorrhagic stroke associated with the highest rates of mortality and long-term neurological disabilities. Despite the improvement in the management of SAH patients and the reduction in case fatality in the last decades, disability and mortality remain high in this population. Brain injury can occur immediately and in the first days after SAH. This early brain injury can be due to physical effects on the brain such as increased intracranial pressure, herniations, intracerebral, intraventricular hemorrhage, and hydrocephalus. After the first 3 days, angiographic cerebral vasospasm (ACV) is a common neurological complication that in severe cases can lead to delayed cerebral ischemia and cerebral infarction. Consequently, the prevention and treatment of ACV continue to be a major goal. However, most treatments for ACV are vasodilators since ACV is due to arterial vasoconstriction. Other targets also have included those directed at the underlying biochemical mechanisms of brain injury such as inflammation and either independently or as a consequence, cerebral microthrombosis, cortical spreading ischemia, blood–brain barrier breakdown, and cerebral ischemia. Unfortunately, no pharmacologic treatment directed at these processes has yet shown efficacy in SAH. Enteral nimodipine and the endovascular treatment of the culprit aneurysm, remain the only treatment options supported by evidence from randomized clinical trials to improve patients’ outcome. Currently, there is no intervention directly developed and approved to target neuroinflammation after SAH. The goal of this review is to provide an overview on anti-inflammatory drugs tested after aneurysmal SAH.
