A prospective randomized comparison of cold vs hot snare polypectomy in the occurrence of postpolypectomy bleeding in small colonic polyps

Abstract: The data show that the CSP technique is safe, effective and quicker than HSP for patients and it could be considered the ideal procedure for small polyp removal.

“…Avoidance of electrocautery in resection of diminutive polyps and some small (6-9 mm) polyps, in favor of cold resection techniques (particularly cold snaring), has proven remarkably safe. 155,156 Submucosal injection likely reduces risk during EMR. A guidewire passed through strictures before an attempt to push an endoscope through can prevent the instrument tip from sliding off the stricture and dissecting the adjacent colon wall.…”

“…No significant difference in terms of polyp removal rates and retrieval was observed between the two methods. Intraprocedural bleeding was more frequent in the cold group in two studies,40,41 but resolved spontaneously and required no additional intervention, while in the other,42 immediate and delayed bleeding was more frequently observed in the hot snare group. Hot snare polypectomy required a longer procedural time and had more post-procedural abdominal symptoms.…”

“…Cold snare polypectomy has been proven to be a safe and effective method for the removal of small colonic polyps without the risk of causing electrocautery burns and perforation (4-6). In addition, there is no difference in the occurrence of post-polypectomy bleeding in small colonic polyps removed by cold snare polypectomy compared with the conventional hot snare polypectomy (7). As a result, cold snare polypectomy for small colonic polyps has been widely used, however, the use of this method for small bowel polyps has not yet been reported.…”
