Sulfur mustard (SM) is a strong blistering, highly reactive, lipophilic chemical war agent that causes injury in different organs including the skin, eyes, and respiratory tract. The Eyes are especially susceptible to the consequences of SM poisoning because of the aqueous and mucosal nature of conjunctiva and cornea. DNA alkylation and depletion of glutathione, are the most important mechanisms of SM action in the eye injuries. Acute clinical symptoms are including decrease in visual acuity, dryness, photophobia, blepharospasm, conjunctivitis, and complaints of foreign body sensation and soreness that gradually progress to severe ocular pain. Corneal abrasions, ulcerations, vesication, and perforations are common corneal consequences in SM injured victims. Appearance of chronic symptoms has been reported as chronic inflammation of the corneal and conjunctival vasculature, ischemia, lipid and cholesterol deposition, scarring in cornea, corneal thinning, opacification and perforation of the cornea, limbal stem cell deficiency (LSCD), and neovascularization. Different medical and surgical protocols have been documented in the management of SM-induced ocular injuries, including preservative-free artificial tears, topical steroids and antibiotic, mydriatic, antiglaucoma drops, therapeutic contact lenses, dark glasses and punctal plugs/cauterization, N-acetylcysteine, tarsorrhaphy, amniotic membrane transplantation, stem cell transplantation, and corneal transplantation. New drugs such as resolvin E1, topical form of essential fatty acids, thymosin β4, 43 amino-acid polypeptides, topical form of curcumin, newly formulated artificial tears, diquafosol, rebamipide, tretinoin, and oral uridineseems to be beneficial in the management of ocular lesion associated with sulfur mustard poisoning. Mustard gas is a lipophilic, highly cytotoxic agent that rapidly penetrates tissue, and the eye is one of the organs mostly affected. Mustard gas-related ocular injuries can be divided into immediate, chronic, and delayed-onset phases. Late complications, developing after 1-40 years, can cause progressive and permanent reduction in visual acuity and even blindness. A wide range of late ocular involvements have been reported, which include chronic blepharitis, limbal ischemia and stem cell deficiency, and corneal scarring and neovascularization. The majority of corneal involvements are limited to the anterior stroma, leaving the posterior stroma and endothelium relatively intact. Therefore, lamellar keratoplasty is appropriate for the management of corneal involvements in the majority of victims. This procedure can be performed alone or in combination with limbal stem cell transplantation.Mustard gas-related ocular injuries can be divided into immediate, chronic, and delayedonset phases [12]. The artificial tears on the basis hyaluronic acid is primary importance today in dry eye syndrome (DES) treatment. In recent years, they have been supplemented Optinol® Express Moisture (0.21 %) and Optinol® Deep Moisture (0.4 %) differing concentrations of sodium hyaluronate and viscosity. The study is devoted to assessing the effectiveness of these drugs in the treatment of patients with dry eye syndrome (DES) of various etiology and severities.There were 73 volunteers with DES of various etiology: 21 — Sjogren’s syndrome, 24 — meibomian blepharitis, and 28 — perimenopause in the study. All patients were divided into 2 groups: 36 patients of the first group received instillations into conjunctival cavity of the Optinol® Express Moisture (0.21 %) and 37 patients of the second group — Optinol® Deep moistening (0.4 %).Starting from the first days of therapy, all patients had a decrease in the severity of subjective signs of DES, estimated by the value of the ocular surface disease index (OSDI). At the same time, tendency to stop of the degenerative changes in the epithelium of the ocular surface was established. It is characterized by a decrease in the degree of staining. The increasement of the tear film stability and an index of the tear meniscus was found in all patients. The dynamics of the controlled parameters increased, as the instillation of the drugs took place and by the 30th day of therapy. There were significant differences from the baseline values. At the same time, the Optinol® Express Moisture (0.21 %) was more effective (mainly in terms of subjective discomfort and the severity of degenerative changes in the epithelium of the ocular surface) in patients with mild and extremely severe form of the xerosis process.The drug Optinol® Deep Moisture (0.4 %) was more effective in patients with xerosis of moderate severity and severe by the same parameters. In the course of research, we have not observed any side effects of both drugs. It allows us to recommend the drugs Optinol® Express Moisture (0.21 %) and Optinol® Deep Moisture (0.4 %) to widespread clinical use in treating patients with DES of various etiology.
