A Solid Pseudopapillary Tumour of the Head of Pancreas: A Rare Case Report Diagnosed by Fine Needle Aspiration Cytology

Abstract: A 17-year-old girl presented with 3-4 months history of upper abdominal pain more on right side. She also had history of postprandial pain, fullness and on and off fever and generalised weakness. Physical examination revealed right hypochondriac tenderness at palpation. Detailed general and systemic examination revealed no other significant findings. The laboratory test including complete blood count, AST, ALT, alkaline phosphatase, bilirubin, random blood sugar and amylase were in normal range. She underwent u… Show more

“…An endoscopic ultrasound (EUS)‐guided needle biopsy or cytology is often performed for the pre‐operative diagnosis of SPNs . When an EUS‐guided needle biopsy is performed the microcystic pattern of SPNs can cause diagnostic confusion, especially when the amount of the provided sample is inadequate.…”

“…The accuracy of EUS with FNA for the diagnosis of SPTs ranges from 41.2% to 69%, 6,18 and is becoming increasingly accepted by surgeons, especially for patients who are difficult to diagnose. 19 However, some authors have argued that EUS with FNA should not be the first choice due to the uncertain potential for malignancy and the high risk of false negatives. 20 Consequently, it is necessary to use a combination of techniques when examining patients.…”

“…Pancreatic solid pseudopapillary neoplasms are distinct histologically [1,3]. Recently, endoscopic ultrasound-guided fine needle aspiration cytology has been an invaluable adjunct [1,3,4,6,8] in pre-and peri-operative tissue diagnosis [6], yielding a highly cellular aspirate, demonstrating numerous pseudorosettes and pseudopapillary branching tissue fronds in multiple layers with a slender fibrovascular core [4,6,8]. However, most cases are diagnosed with full tissue histology with the characteristic appearance similar to our patient as uniform, bland ovoid cellular architecture with eosinophilic cytoplasm, pseudopapillae with hyalinized cores, clusters of cholesterol clefts, foamy cells, and multinucleated giant cells [4,9].…”
