Accessory heads of the biceps brachii muscle: A systematic review and meta‐analysis

The anatomy of the biceps brachii muscle has been a subject of interest to many researchers. In particular, the presence of one or more accessory heads has been reported to be the most common variation of the biceps brachii muscle. In fact, contemporary knowledge is quite inconsistent and lacks a definitive summary. Taking this into account, the present study aims to investigate the overall prevalence of the accessory heads and related questions, such as their broad morphological features, population variance, sexual dimorphism, side distribution or the changes in prevalence rates over time. A literature search of major scientific databases was conducted and produced 78 eligible articles, including 10,603 upper limbs, for our study. Relevant data were extracted and consequently analysed with the use of random-effects metaanalysis. As a result, the accessory heads occur with an overall prevalence of 9.6% (95% CI 8-11%) and by far the most common is the presence of a single accessory head (8.4%; 95% CI 7-10%). Additional sub-analyses revealed that accessory heads appear more frequently unilaterally and in males. Differences between the occurrence on the right or left side were not significant. Moreover, a decreasing trend in prevalence rates over time was observed, pointing towards an evolutionary adaptation. The innervation and blood supply of the accessory heads are nearly identical to that of the normal biceps brachii muscle. Although the accessory heads are usually asymptomatic, their potential presence must be kept in mind while interpreting various conditions. In addition, their direct clinical importance is speculated and is thought that they might be conductive to several pathological processes around the shoulder girdle and brachial region. While many of the morphological and morphometric aspects of the accessory heads are well documented, their functional value is still a matter for future investigations.

Background. The biceps muscle has proximal tendons that attach the muscle to the shoulder and one distal tendon that attaches at the elbow, called the distal biceps tendon. Normal morphometric measurements of the distal biceps tendon serve as an important landmark in restoring its biomechanical characteristics and anthropometric evaluation during surgical tendon repairs. Hence the objective of this study was to provide a detailed morphometric profile of the distal biceps tendon. Methods. A total of 50 dissected adult limbs were studied. Out of the 50 limbs, 25 belonged to the right side while 25 were of the left side. The insertion pattern of the distal tendon was dissected and recorded. Total length, post aponeurotic length, the proximal, and distal thickness of the tendon was measured. Results were tabulated and correlated using SPSS. Results. The mean length of the tendon was 8 cm. The proximal thickness varied from 2 to 4 cm, distal thickness from 1 to 3 cm. Tendon length and thickness showed no statistically significant differences between the right and left sides. However, significant correlation (p < 0.001) was observed between length and thickness, proximal and distal thickness of the tendon. Conclusions. The variation in length, width of distal biceps footprints and radial tuberosities may be significant in the anatomical restoration of the distal biceps tendon. This morphometric study of distal biceps tendon would be beneficial to sports medicine physicians for diagnosis and treatment of biceps tendon tear and tendinopathy.

ResumenLa variable más común del músculo biceps brachii es la presencia de cabezas supernumerarias. El objetivo del presente reporte de caso es describir la presencia de una tercera cabeza del músculo biceps brachii y su significancia clínica. Las variaciones anatómicas descritas fueron encontradas durante una disección de rutina realizada en el laboratorio de morfología de la Universidad de Pamplona en dos cadáveres masculinos, embalsamados, de 75 y 59 años de edad, respectivamente. Las medidas fueron tomadas usando un calibrador Vernier. Los detalles topográficos de las variaciones fueron examinados, registrados y fotografiados. Han sido descritas compresiones de la arteria braquial, nervio mediano, nervio musculocutáneo entre las cabezas supernumerarias del músculo biceps brachii, su conocimiento es de interés para clínicos, especialistas y profesionales de la salud, en aras de lograr diagnósticos, procedimientos y rehabilitaciones más seguras y eficientes.Palabras clave: Músculo biceps brachii, Cabeza supernumeraria, Variación anatómica. AbstractThe most common variable of the muscle biceps brachii is the presence of supernumerary heads. Measurements were taken using a Vernier caliper. Topographic details of the variations were examined, recorded and photographed. They have been described compression of the brachial artery, median nerve, musculocutaneous nerve between the supernumerary heads of muscle biceps brachii, your knowledge is of interest to clinicians, specialists; and health professionals in order to achieve diagnoses, procedures and safer and efficient rehabilitations.
