Background Tuberculosis (TB) has long been associated with negative attitudes among people as a contagious, incurable and emaciating disorder. This study aimed to assess the prevalence of stigma towards tuberculosis, among patients with pulmonary tuberculosis. Methods A descriptive cross-sectional study was carried out at the Chest Clinic, Matara among 116 smear positive pulmonary tuberculosis patients. Self-administered questionnaire and indepth interviews were carried out to collect data and multiple variables were analyzed. Results Of the 116 patients 62(53%) were male and 54(47%) were female. Sixty (52%) were employed. Mean age was 47.84 years and age range 12-89 years. Analysis of education level indicated that 2(1.7%) had pre-primary, 49(42.2%) primary, 48(41.4%) lower secondary level, 15(12.9%) upper secondary and two (1.8%) post-secondary education. The four categories of stigma; external stigma, internalized stigma, anticipated stigma and courtesy stigma were assessed. With regard to external stigma, 68 (58.6%) were reluctant to investigate close contacts, 10(16%) lost their jobs and in two (16%), partners refused to marry. With regard to internalized stigma, 82 (70.7%) were reluctant to obtain direct observer treatment from a person in the village and 74(64%) had had a mental setback. In anticipated stigma, 94(81%) maintained a distance when talking to others and 89 (76.7%) were reluctant to disclose their status to persons other than close family members. Conclusion This study shows stigmatization towards the disease in many aspects. Findings could be utilized to implement multidimensional stigma mitigation interventions including advocacy programs, social mobilization and individual behavioural therapy. .

Introducción. El estigma hacia la tuberculosis afecta la búsqueda de atención médica oportuna y la adherencia al tratamiento por temor al rechazo familiar, social o institucional. Objetivo. Validar un instrumento para medir el estigma hacia la tuberculosis aplicando la metodología Rasch. Métodos: Estudio de corte transversal. El análisis se basó en los aspectos sugeridos por Messick y operacionalizados por Wolfe y Smith con la metodología Rasch; se empleó el modelo dicotómico, donde se evaluaron los estimados de ajuste de los ítems, coherencia en la medida, índices de separación y confiabilidad, invarianza, unidimensionalidad y la relación persona-ítem. Resultados. Se entrevistaron 250 personas, 195 sin tuberculosis y 55 con tuberculosis. Se evaluaron 35 ítems, de los que 10 fueron excluidos por presentar desajustes y funcionamiento diferencial del ítem. Se obtuvieron 25 ítems validos con ajustes apropiados entre 0,5 y 1,5, correlación >0,3, porcentaje de coherencia >40% e invarianza en la medición de importantes variables exógenas. El índice de separación en ítems fue de 6,5 y confiabilidad de 0,98. En las personas fue de 1,51 y 0,69 respectivamente. Conclusiones. Se obtuvo un instrumento válido con medida unidimensional del estigma hacia la tuberculosis para uso en población general y en pacientes con tuberculosis, soportado en la rigurosidad estadística del modelo Rasch.

Tuberculosis and the sexual and reproductive lives of women in Bangladesh

When they do not meet norms related to sexuality and reproduction, Bangladeshi women often face abandonment and are thus deprived of an active sexual life, a marital relationship, and motherhood. Little is known about how a stigmatised disease such as tuberculosis (TB) may constrain the reproductive health and sexual lives of women. This article, derived from a larger study on the impact of TB on women’s sexual and reproductive health and rights in Narsingdi district and Dhaka, Bangladesh, aims to fill this gap. Based on interviews with nine married women who have or had TB, four husbands, and two mothers-in-law, this article highlights that the ways in which TB impedes on the sexual and reproductive lives of women depends on the stigma within their family and community, their relationships with their husbands, motherhood, their living arrangements, their economic contribution to the family and/or their disclosure of their TB diagnosis. Women with children and supportive husbands retain a stronger position among their in-laws and are less likely to be isolated or rejected. The patients’ narratives revealed that the instructions of health workers influenced their decisions about intercourse or abstinence. Future studies should examine the instructions patients receive from health workers regarding their living and sleeping arrangements, sexual intercourse, and pregnancy, as well as policy documents on TB treatment and prevention.
