A suprasellar arachnoid cyst resulting from an intraventricular haemorrhage and showing complete resolution following endoscopic fenestration

Abstract: SUMMARYWe present the case of a premature neonate who developed a large, acquired arachnoid cyst as a consequence of intraventricular haemorrhage. The child was managed with endoscopic fenestration and made an excellent recovery. BACKGROUND

“… 11 Several reports of de novo arachnoid cysts have been described following trauma and/or intraventricular hemorrhage of prematurity. 5 , 15 , 16 It is thought that this type of cerebral trauma may lead to microsplitting of the arachnoid membranes, allowing CSF accumulation. 5 Our patient did not experience either trauma or intraventricular hemorrhage of prematurity.…”

“…Other etiologies in the literature illustrate de novo cyst formation from head trauma or intraventricular hemorrhage of prematurity. [ 8 , 9 , 14 ] Enlargement, which occurs in an unpredictable manner, is thought to occur by either active fluid secretion from cyst walls, expansion from osmotic gradients, or unidirectional valve mechanisms. [ 3 , 6 , 10 ]…”

“…Interestingly, there are rare exemplary case descriptions in the literature illustrating de novo cyst formation from head trauma or intraventricular hemorrhage of prematurity. [ 8 , 9 , 14 ] The prevalence of these lesions has incidentally increased with rising utilization of imaging modalities, namely, computed tomography and magnetic resonant imaging (MRI). The majority of these cysts are asymptomatic, and their long-term behavior is often unknown.…”

“…treatment of various neurosurgical diseases. Especially in the past 10 years, neuroendoscopy has been rapidly popularized in clinical applications because of the improvement of endoscopic surgical instruments and surgical techniques [6][7][8]. Neuroendoscopy has a short focal length, wide field of vision, good lighting and image quality and can be used in operations such as electrocoagulation hemostasis, irrigation, and biopsy by using different channels in the endoscope [9].…”
