A Virtual Ward for Home Hemodialysis Patients – A Pilot Trial

Abstract: BackgroundPatients with end-stage renal disease (ESRD) have a high rate of hospitalization and are prone to care gaps that may occur during the transition from hospital to home. The virtual ward (VW) is an innovative model that provides short-term transitional care to patients upon hospital discharge. The VW may be an effective intervention to address care gaps.ObjectivesThe primary objective of the pilot study was to assess the feasibility and practicality of implementing the Home Dialysis VW (HDVW) on a broa… Show more

“…Patients newly initiating home dialysis are at high risk of experiencing gaps in care and interventions to improve communication are potentially best used at these vulnerable times. 27 Due to the importance of needing a baseline patient experience with home dialysis care to assess improvement after initiating the portal, our study was structured to only include prevalent patients who were established on home dialysis. Many of these patients may have been well connected to the health care team and less likely to use or benefit from a change in communication.…”

“…Acute transfer may also lead to care gaps such that medical data transfer may be suboptimal between the PD and HD teams. 4 Moreover, patients transferring to HD most often initiate HD with a central venous catheter, exposing them to heightened infectious and cardiovascular risk. 5 Recent data also suggest that risk for adverse events, including death, may be higher after a modality switch, similar to the initial transition to renal replacement therapy.…”
