Ab interno trabeculectomy: Ultrastructural evidence and early tissue response in a human eye

Abstract: Observations confirm that ab interno trabeculectomy causes direct communication between Schlemm's canal lumen and the anterior chamber in vivo and immediately after enucleation during the early postoperative period. The absence of an evident inflammatory reaction in the examined case should be considered with caution because of possible tumor-induced immune suppression.

“…Histologically, both the trabecular meshwork and the inner wall of Schlemm canal are removed with this technique resulting in a direct communication between Schlemm canal lumen and the anterior chamber. 20 In a small prospective case series of 11 eyes with medically uncontrolled primary open angle glaucoma (9) or pseudoexfoliation glaucoma (2) and formerly unsuccessful filtering surgery (trabeculectomy, nonpenetrating deep sclerotomy, or viscocanalostomy), ab interno trabeculectomy was combined with phacoemulsification. One of the 11 patients ''failed'' and required additional surgery for control of intraocular pressure.…”
