ABCD3-I score and the risk of early or 3-month stroke recurrence in tissue- and time-based definitions of TIA and minor stroke

Abstract: Changing definition of TIA from time to a tissue basis questions the validity of the well-established ABCD3-I risk score for recurrent ischemic cerebrovascular events. We analyzed patients with ischemic stroke with mild neurological symptoms arriving < 24 h after symptom onset in a phase where it is unclear, if the event turns out to be a TIA or minor stroke, in the prospective multi-center Austrian Stroke Unit Registry. Patients were retrospectively categorized according to a time-based (symptom duration belo… Show more

“…The term “transient neurological symptoms” is used to describe a broad spectrum of symptoms following TIA or TIA mimics (20). Unilateral weakness and speech disturbances are the most common clinical manifestations of TIA, and our findings were in accordance with the prior studies (21, 22). It was reported unilateral weakness and speech disturbance were found in ~31–54% and 25–42% of TIA, respectively (5).…”

“…In the population of Spain, the area under the ROC curve of ABCD2 and ABCD3-I for ischemic events prediction were 0.49 and 0.77 [31]. Recently, the ABCD3-I score performed equally in TIA patients in tissue-as well as time-based definition and the same for minor stroke patients [22]. Our study was also in line with the above studies.…”

“…The term of 'transient neurological symptoms' is used to describe the broad spectrum of symptoms following TIA or TIA mimics. Unilateral weakness and speech disturbances are the most common manifestations of TIA [22]. It was reported unilateral weakness and speech disturbance were found in approximately 31%-54% and 25%-42% of TIA, respectively [18].…”

“…These new scoring systems have been well validated in other populations, reporting the c-statistics of 0.60–0.64 in predicting the recurrent stroke within 3 month following TIA. However, new scores such as ABCD 3 -I score have not been recommended for risk stratification in such patients by the guidelines by far (10, 11).…”
