Abducens Nerve Palsy Associated With Pseudomeningocele After Lumbar Disc Surgery

Abstract: Although uncommon, abducens nerve palsy after cerebrospinal fluid leak should be considered in the differential diagnosis of diplopia developing in a patient who has undergone spine surgery. After confirmation of pseudomeningocele radiologically, early surgical intervention with repair of the dural defect can result in complete recovery of the abducens nerve palsy.

“…A recent, review guide devoted to the treatment of spontaneous intracranial hypotension recommended fibrin sealant as one method to close an identified leak [227]. A case report used fibrin sealant to help seal a dural CSF leak causing a pseudomeningocele following lumbar disc surgery with a subsequent complete resolution of a concomitant abducens nerve palsy and diplopia [228]. Several papers address the role of fibrin sealant in nerve repair including an end to side nerve repair in rats [229] and a facial nerve repair in dogs [230] failing to find a fibrin sealant benefit and a favorable report of using a fibrin conduit for mesenchymal stem cells particularly in the presence of cylcosporine to enhance regeneration following peripheral nerve injury in rats [231].…”

“…If handled improperly, it may lead to a number of adverse sequelae, such as CSF fistulas (persistent leak through the incision), meningitis, brain abscess, intracranial hemorrhage, hematoma, and neurological deficits 1, 2, 3, 4. Literature review was conducted from the databases of PubMed, Ovid, and Elsevier.…”

“…It should be noted, however, that it is very rare to see paralysis of the sixth cranial nerve as a complication of spine surgery [11]. Barsoum et al described some cases of cranial nerve VI paralysis related to traction during the procedure [12], and Abd-Elsayed et al [13] reported paralysis related to facial swelling caused by the intra-operative positioning of the patient.…”
