Ablation of perimitral flutter: acute and long-term success of the modified anterior line

Abstract: The MAL is effective for acute and long-term treatment of PMFL. Maintenance of bidirectional MAL block was shown in 58% of patients during a repeat ablation.

“…In case of a roof‐dependent macroreentry, a roof line was placed. In case of perimitral flutter, a modified anterior line was placed as previously described . The endpoint for both lines was bidirectional block confirmed by differential pacing and the presence of widely split double potentials on the ablation line.…”

“…For substrate modification, electrogram guided ablation was performed with the endpoint AF CL prolongation or AF termination (into AT or sinus rhythm), as described before . Linear lesions including a modified anterior line and/or a roofline were deployed in case of extensive LVA on the anterior wall and/or roof, or in case of subsequent macroreentries . If sinus rhythm was not achieved after these steps, direct current cardioversion was performed at the end of the procedure.…”

“…At the end of the procedure in all patients, a complete pulmonary vein isolation and in case of additional linear lesions using differential pacing a bidirectional block of all lines could be confirmed. 13…”

“…The roof line was deployed between both upper pulmonary veins. A modified anterior line (MAL) was performed as described earlier by our group . Briefly, RF ablation was started with an atrioventricular electrogram ratio of 1:2 at the anterior part of the mitral annulus.…”

“…All patients who did not convert in SR after ablation underwent a DCC at the end of the procedure. In SR, all veins were checked again to confirm isolation and LL were tested for bidirectional block using differential pacing as described before . Briefly, the MAL conduction block was assessed by pacing from the LAA and septally to the line while observing CS activation sequence, and activation times between the CS and LAA, respectively.…”
