ABM Clinical Protocol #32: Management of Hyperlactation

Abstract: A central goal of the Academy of Breastfeeding Medicine is the development of clinical protocols for managing common medical problems that may impact breastfeeding success. These protocols serve only as guidelines for the care of breast-feeding mothers and infants and do not delineate an exclusive course of treatment or serve as standards of medical care. Variations in treatment may be appropriate according to the needs of an individual patient.

“…Birth control pills delivering E + P decreased the content of LALBA in breast milk and overall milk production, although the volumetric decrease was still within the normal range of output [80]. Birth control pills containing E are also used to treat hyperlactation in humans, although the mechanism is undefined [81]. In lactating cows, a single dose of synthetic E accelerated mammary involution coincident with a reduced concentration of LALBA and lactose in milk following final milk removal [82,83].…”

“…We advocate that patients with galactoceles undergo stab incision and drain placement by surgeons, or drain placement by interventional radiology for definitive resolution of the galactocele. [9][10][11] Physicians may wish to utilize betadine for flushing an infected fluid cavity; however, this should not Soares • Management of Infected Breast Implant used in lactating breasts because the levels that concentrate in breastmilk are unsafe for the infant. 12 This patient developed her implant infection and abscess cavity surrounding the implant as a result of repeated instrumentation and incomplete drainage of her infected galactocele.…”
