Abnormal lung ultrasound pattern during labor: A prospective cohort pilot study

Abstract: Purpose Lung ultrasound (LUS) examination is used to evaluate patients with acute respiratory failure. The physiological LUS pattern during labor in healthy parturients has not been well described. The aim of this study was to evaluate the LUS pattern in a cohort of healthy women during uncomplicated labor. Methods We used the 8‐point LUS assessment protocol and investigated lung sliding, A‐lines, B‐lines, interstitial syndrome, lung consolidation, and pleural effusion according to the International Consensus … Show more

“…This study also showed that the most common ultrasound pattern in pregnant women is the A-profile. On the contrary, Krawczyc et al reported that lung ultrasound (LUS) was abnormal in at least one region-showing three or more B-lines-in 21% of their population of women during labor [30]. Although this was a small study involving just 24 women with uncomplicated labor, it reminds us that a basal examination is of paramount importance for detecting changes during labor and for being able to identify the correct therapy.…”

“…Previous studies documented how diffuse B-lines may be present in asymptomatic parturients. 11,30 The finding of frequent atelectasis in this group may be explained by the exposure to risk factors such as higher BMI and general anesthesia. 27,31 Pulmonary complications were significantly associated with poorer outcome, confirming findings in non-obstetric patients in Africa 12 and Asia.…”

“…Limited data have been available on the prevalence of pulmonary interstitial syndrome in women undergoing a VD 19 or CD. 20 Findings from our study that followed international consensus recommendations on performing LUS 10 provide important prevalence data in the immediate postpartum setting.…”

“…With diuretic treatment and improving symptoms, a normalization of LUS pattern has been observed, indicating that LUS does well reflect pulmonary function. 11,16 Krawczyk et al 19 found pulmonary interstitial syndrome to be present in 5 of 24 (21%) of women when LUS was performed during labor and within 2 hours following a VD. In contrast, no women who underwent VD in our cohort developed pulmonary interstitial syndrome.…”
