Accelerated Partial Breast Irradiation: A Review and Description of an Early North American Surgical Experience with the Intrabeam Delivery System

Abstract: While a variety of APBI techniques are currently available for clinical use, our early North American operative experience with IORT shows it is well tolerated with low morbidity. Delivery of IORT adds moderate operative time and may require creating subcutaneous tissue fl aps. The addition of WBI may be necessary in situations for positive residual margins or microscopic nodal disease in patients who do not undergo additional surgery.

“…At the present time, external beam IORT is administered by a dedicated linear accelerator with parallel electron beams of 3–12 MeV kinetic energy or isotropic photon fields with energies between contact and superficial therapy of 50 kV X-rays (38). Such low photon energy beams (e.g., Intrabeam ® by Carl Zeiss AG, Germany) have demonstrated favorable outcomes in some sites but with many questions remaining unanswered (39, 40). …”

“…The surgical technique for IORT with the Intrabeam system has been previously described. 9 Briefly, all patients underwent preoperative tumor localization by either ultrasound or stereotactic guidance, and radiotracer injection for sentinel lymph node biopsy (SLNB). All patients received a single dose of preoperative antibiotics.…”

“…The lymph nodes (LN) can function as “filters” to remove cancer cells from the systemic circulatory system. The regional immune responses within LNs during cancer progression can be an important prognostic indicator and LN response often serves as a critical criterion for evaluating therapeutic response(Deneve et al, 2012; Lee et al, 2012),(Kimber et al, 2000; Helle et al, 2012; Noh et al, 2012). Infection and/or tumor cell migration commonly leads to increases in LN size.…”
