Accuracy of human papillomavirus tests on self-collected urine versus clinician-collected samples for the detection of cervical precancer: a systematic review and meta-analysis

Abstract: Objective The human papillomavirus (HPV) test is an effective screening tool to prevent cervical cancer. Urinary sampling for HPV detection improves the accessibility and participation of screening services and reduces the cost and burden on physicians. The clinical accuracy of urinary HPV test has yet to be determined via meta-analysis. This study assessed the clinical accuracy of these tests to detect cervical intraepithelial neoplasia (CIN) 2 or worse. Methods Releva… Show more

“…12 , 13 , 14 It has already been shown that HPV testing on self-collected samples has a high acceptance rate, but HPV testing in urine, being thoroughly non-invasive, goes one step further and has been demonstrated to be a reliable sampling method in individuals with a cervix. 7 , 16 , 17 In this study, not only was the willingness to participate impressively high (only 3 out of 203 invited subjects declined to participate), but also the subjects’ appreciation that their medical problems were being recognized was remarkable.…”

“…Devices for self-collected urine are licensed gender-neutral, have a high acceptance, and the same HPV testing accuracy as clinician-collected samples in people with a cervix. 16 , 17 …”

“…Future research is needed after awareness campaigns to evaluate the effectiveness of the intervention and awareness plans. In a recent meta-analysis, it was shown that self-collected urinary HPV tests have similar accuracy to the cervical HPV test in detecting CIN2 or worse disease stages [ 38 ]. Interestingly, the sensitivity of urine HPV was higher in low- and mid-income countries [ 38 ].…”

“…In a recent meta-analysis, it was shown that self-collected urinary HPV tests have similar accuracy to the cervical HPV test in detecting CIN2 or worse disease stages [ 38 ]. Interestingly, the sensitivity of urine HPV was higher in low- and mid-income countries [ 38 ]. This highlights the necessity of adopting it as a screening technique for the early diagnosis of cervical pre-cancer, particularly in marginalized populations with limited access to screening units.…”

“… 9 The HPV test is an efficient screening strategy to prevent CC. 10 Nonetheless, some CC patients have false-negative HR-HPV test results, indicating the limitation of HPV screening. 11 , 12 Hence, finding novel and valid biomarkers of HR-HPV infection is of great magnitude for the initial diagnosis and treatment of CC.…”
