Objective: Antibiotic resistance development in the treatment of acinetobacter infection is a serious health care problem and responsible for high mortality in intensive care units (ICU). In our study, it was aimed to determine rates of antibiotics resistance of Acinetobacter strains isolated from various samples in our ICU. Methods: We examined the records of acinetobacter isolates and antibiotics resistance for one year follewed up in our ICU. The samples from different patients and different type of samples of the same patients were evaluated.The data was analyzed with SPSS for Windows version 23.0. Categorical variables were expressed in terms of numbers and percentage. Results: 50% of the samples were isolated from tracheostomy.96.4% of the 138 isolates were A. Baumanii and 3.6% were the other strains.We found high resistance to all of antibiotics except colistin(3.6%) and tigecycline (13.1%). Conclusion: Acinetobacter is the most important opportunistic human pathogen causing fatal nosocomial infections because of its ability of develop resistance new antibiotics overly fast. Compared to the results reported from Dicle University Hospital in south east of our country it seems that antibiotics especially cholistin resistance ratio in our ICU is different. It is important to remember that antibiotic susceptibility may vary in regions, hospitals even clinics, and resistance development should be constantly detected to make the appropriate initial therapy until deescalation.

ÖZ Amaç: Bu çalışmada, yoğun bakım ünitesi (YBÜ) örneklerinden izole edilen Pseudomonas aeruginosa ve Acinetobacter baumannii suşlarının üç yıllık antibiyotik duyarlılıklarının incelenerek direnç oranlarındaki değişimin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ocak 2015-2018 arasında YBÜ örneklerinden izole edilen Pseudomonas aeruginosa ve Acinetobacter baumannii suşlarının direnç profilleri retrospektif olarak incelendi ve yıllık direnç oranları değerlendirildi. Bulgular: Üç yıllık süre içinde YBÜ örneklerinin 609'unda Acinetobacter baumannii, 420'sinde Pseudomonas aeruginosa izole edilmiştir. İzolatların en sık Genel YBÜ'den izole edildiği saptanmıştır. Acinetobacter baumanni'nin kolistin, amikasin, gentamisin, imipenem, meropenem, siprofloksasin, seftazidim, piperasilin/tazobaktama 3 yıllık ortalama direnci sırasıyla %0,5, %79, %86, %96, %96, %97, %98, %99, Pseudomonas aeruginosa'nın ise sırasıyla %5, %21, %25, %35, %38, %40, %42, %45, %49, %53 olarak tespit edilmiştir. Acinetobacter baumannii ve Pseudomonas aeruginosa suşlarında en etkili antibiyotiklerin sırasıyla kolistin (%99,5, %95) ve amikasin (%21, %79) olduğu görülmüştür. Sonuç: Hastanemiz YBÜ'lerinden izole edilen Acinetobacter baumannii izolatlarının birçok antibiyotiğe dirençli olduğu, Pseudomonas aeruginosa izolatlarının daha duyarlı olduğu saptanmıştır. Bu veriler, Acinetobacter baumannii kaynaklı enfeksiyonların tedavisinde yeni antibiyotiklere ihtiyaç duyulduğunun kanıtı niteliğindedir. Her hastanenin kendi direnç oranlarındaki değişimleri takip etmesi ve ampirik tedavi ilkelerinin güncellemesinin uygun olacaktır. Anahtar Kelimeler: Acinetobacter baumannii, Pseudomonas aeruginosa, antimikrobiyal direnç, yoğun bakım ünitesi ABSTRACT Objective: The objective of the study was to investigate the three-year antibiotic susceptibility of Pseudomonas aeruginosa and Acinetobacter baumannii strains isolated from intensive care unit (ICU) specimens and to evaluate the changes in resistance rates. Materials and Methods: The resistance profiles of Pseudomonas aeruginosa and Acinetobacter baumannii strains isolated from ICU specimens between January 2015-2018 were analyzed retrospectively and the annual resistance rates were evaluated. Results: Acinetobacter baumannii was isolated in 609 and Pseudomonas aeruginosa was isolated in 420 of the ICU specimens over a three-year period. Strains were most frequently isolated from general ICU. The three-year mean resistance of Acinetobacter baumannii and Pseudomonas aeruginosa to colistin, amikacin, gentamycin, imipenem, meropenem, ciprofloxacin, ceftazidime, piperacillin / tazobactam was 0.5%, 79%, 86%, 96%, 96%, 97% 98%, 99% and 5%, 21%, 25%, 35%, 38%, 40%, 42%, 45%, 49% and 53%, respectively. Amaç: Antibiyotik direnci son yıllarda tüm dünyada artış göstermiştir. Bu durum özellikle yoğun bakım ünitelerinde (YBÜ) yatan hastalarda ciddi sağlık problemi oluşturmaktadır. Acinetobacter baumannii YBÜ’de yatan hastalarda çoklu ilaç dirençli enfeksiyonların önemli bir nedeni haline gelmiştir. Bu çalışmanın amacı YBÜ’den izole edilen A.baumannii suşlarının direnç oranlarını saptamaktır. Gereç ve Yöntem:2015-2019 yılları arasında YBÜ’lerden mikrobiyoloji laboratuvarına gönderilen çeşitli örneklerden izole edilen A.baumannii suşları çalışmaya dahil edilmiştir. Bakteri tanımlaması ve antibiyotik duyarlılık testleri konvansiyonel yöntemler ve otomatize sistemler kullanılarak yapılmıştır. Bulgular: Toplam 773 A.baumannii suşu çalışmaya dahil edilmiştir. Suşların izole edildiği örnekler arasında endotrakeal aspirat örnekleri ilk sırada (%65.3) saptanmıştır. İzole edilen A.baumannii suşlarının en dirençli olduğu antibiyotik grubu karbapenemler (%96.5) olarak bulunmuştur. Siprofloksasine %96.1, netilmisine %89.8, gentamisine %88, amikasine %66 ve trimetoprim-sülfametoksazole %67.7 direnç tespit edilmiştir. Ayrıca, direnç oranlarının yıllar içinde arttığı gözlenmiştir. Sonuç: A.baumannii suşlarının antimikrobiyal ajanlara direnç oranları hastaneler arasında farklılık gösterdiğinden her hastane belirli aralıklarla kendi antibiyotik direnç profilini gözden geçirmeli ve antibiyotik kullanım politikalarını belirlemelidir. Ayrıca geniş spektrumlu antibiyotiklerin yaygın kullanımının kısıtlanarak direnç gelişiminin önlenmesi hedeflenmelidir.
