OBJECTIVE: In vocally healthy children and adults, speaking voice loudness differences can significantly confound acoustic perturbation measurements. This study examines the effects of voice sound pressure level (SPL) on jitter, shimmer, and harmonics-to-noise ratio (HNR) in adults with voice disorders and a control group with normal vocal status. STUDY DESIGN: This is a matched case-control study. METHODS: We assessed 58 adult female voice patients matched according to approximate age and occupation with 58 vocally healthy women. Diagnoses included vocal fold nodules (n = 39, 67.2%), polyps (n = 5, 8.6%), and muscle tension dysphonia (n = 14, 24.1%). All participants sustained the vowel /a/ at soft, comfortable, and loud phonation levels. Acoustic voice SPL, jitter, shimmer, and HNR were computed using Praat. The effects of loudness condition, voice SPL, pathology, differential diagnosis, age, and professional voice use level on acoustic perturbation measures were assessed using linear mixed models and Wilcoxon signed rank tests. RESULTS: In both patient and normative control groups, increasing voice SPL correlated significantly (P < 0.001) with decreased jitter and shimmer, and increased HNR. Voice pathology and differential diagnosis were not linked to systematically higher jitter and shimmer. HNR levels, however, were statistically higher in the patient group than in the control group at comfortable phonation levels. Professional voice use level had a significant effect (P < 0.05) on jitter, shimmer, and HNR. CONCLUSIONS: The clinical value of acoustic jitter, shimmer, and HNR may be limited if speaking voice SPL and professional voice use level effects are not controlled for. Future studies are warranted to investigate whether perturbation measures are useful clinical outcome metrics when controlling for these effects. OBJETIVO: Verificar o tempo ideal de execução da técnica de vibração sonorizada de língua necessário para interferir nas medidas de perturbação (do ciclo vibratório das pregas vocais) a curto prazo, de frequência fundamental (F0) e intensidade vocal. MÉTODOS: Foram analisadas 43 voluntárias do sexo feminino, na faixa etária de 18 a 31 anos (média de 21,2). Elas foram submetidas a uma triagem realizada por uma fonoaudióloga e por uma médica otorrinolaringologista, para eliminação de possível patologia laríngea e/ou alteração vocal. Foi realizada a gravação de um [a] prolongado, antes da realização da técnica de vibração sonorizada de língua; e depois do primeiro, terceiro e quinto minuto de realização da mesma. Após a coleta dos dados foi realizada a análise acústica, através do software Vox Metria® 1.1, na qual se analisou a F0 média e sua variabilidade, intensidade média e medidas de perturbação a curto prazo. RESULTADOS: Houve aumento na F0 a partir de três minutos da execução da técnica e na intensidade a partir de um minuto. Houve diminuição do ruído a partir de três minutos de realização da técnica. Os parâmetros de variabilidade da F0, jitter, shimmer, nível de ruído glótico e irregularidade não apresentaram diferença entre os períodos comparados. CONCLUSÃO: A técnica de vibração sonorizada de língua foi mais efetiva a partir de três minutos. Os resultados obtidos confirmaram a eficácia da técnica, a qual é vastamente empregada na prática fonoaudiológica, delimitando o tempo ideal para a realização da mesma.

OBJETIVO: Verificar a ocorrência de queixas e sintomas vocais em atores profissionais de teatro e sua relação com aspectos ambientais desta atividade laboral e com o histórico de intervenções especializadas. MÉTODOS: Estudo transversal com 48 atores profissionais realizada a partir de questionário sobre sintomas e queixas vocais e extra-vocais, bem como aspectos ambientais, hábitos e demandas relacionados ao trabalho de ator de teatro. Foi utilizado protocolo padronizado específico previamente estruturado (PROTEA). RESULTADOS: O histórico de treinamento vocal para atuar foi referido por 40 (83,3%) entrevistados. Os recursos mais utilizados foram aquecimento vocal, hidratoterapia e métodos alternativos. Dificuldades para a manutenção da qualidade vocal nas demandas do dia-a-dia foram referidas por 14 (35,0%) sujeitos, sendo que oito (16,7%) as percebem desde o início da carreira. Durante a atuação, 14 (29,2%) referem dificuldades de coordenação entre fala e respiração. Todos os entrevistados consideraram que a saúde vocal é importante para o ator de teatro. A maioria dos atores negou dificuldades quanto à projeção vocal, respiração e/ou articulação das palavras em cena. Entretanto, o palco italiano parece estar mais relacionado que o teatro de arena com dificuldades de coordenação pneumofonatória (p=0,00). CONCLUSÃO: Em que se pese que atores profissionais têm histórico de treinamento e orientação para a utilização da voz profissional, há porção significativa destes com queixas em relação ao uso profissional da voz, especialmente relacionadas a condições físicas presentes no ambiente de trabalho.
