Acquired Weakness, Handgrip Strength, and Mortality in Critically Ill Patients

Abstract: ICUAP is independently associated with increased hospital mortality. Handgrip strength is also independently associated with poor hospital outcome and may serve as a simple test to identify ICUAP. Clinical trial registered with www.clinicaltrials.gov (NCT00106665).

“…Eleven out of the 29 (38%) 25,28,29,32,[36][37][38][39][40][41][42] observational studies were graded of low quality, 17 out of 29 (59%) 12,[14][15][16]20,24,26,27,30,[43][44][45][46][47][48][49][50] as medium and one (3%) 51 as high quality. There were four RCTs included; two studies were deemed of low risk, 22,23 one unclear risk 8 and one with high risk 31 of bias.…”

“…Malnutrition progresses due to catabolic processes, increasing susceptibility to infection and resulting in a deterioration of physiological function. Then, it leads to an increase in the frequency of infections, duration of ventilation, the length of hospital stay, and mortality rate 285. Appropriate nutritional intervention has been demonstrated to control these vital reactions and improve prognosis 286…”

“…23,26 In this context, the manual dynamometer is a useful tool for identifying patients with muscular weakness, as it presents a correlation with overall muscular strength, as well as a correlation with mortality. 2,3,27 It is important to note that strength deficit is not the only determinant of worsening physical performance, as there are other systems involved. 22 The study has some limitations such as the fact that it is a cross-sectional study, therefore making it impossible to associate the evaluation of these variables over time.…”
