Acute aortic dissection and pregnancy: Review and meta‐analysis of incidence, presentation, and pathologic substrates

Abstract: Objectives: Pregnancy has been recognized as a predisposing factor for acute aortic dissection (AAD) although its occurrence is quite rare. Currently, no trial and few prospective studies exist about this catastrophic event. The present review and metaanalysis aims to update information on clinical presentation, potential risk factors, treatment, and outcome of acute dissection during pregnancy and puerperium. Methods: A comprehensive search of three databases was performed to identify all patients reported in … Show more

“…Reassuringly, we found no association between having MFS and experiencing preterm births. This was unexpected since connective tissue disorders are usually associated with this condition (De Martino et al, 2019; Kernell et al, 2017). Since preterm birth was not even increased in undiagnosed women with MFS, any preconception advice against pregnancy in the most severe cases with MFS cannot have influenced the results.…”

“…For this reason, it is difficult to find a safe threshold and it is necessary to have a closer follow-up in these patients at particular risk. 3,7 Pyeritz, and Smith and Gros reported a negligible risk of cardiovascular complications in patients with MS and ascending aorta size < 40 mm. In the literature, the risk of dissection and rupture in MS is 1% when the aortic diameter is <40 mm 9,10 European guidelines on the management of ascending aortic disease in patients with MS recommend avoiding pregnancy or replacing the ascending aorta before when the aortic diameter is >45 mm or >40 mm if associated with a risk factor.…”

“…And the main feature of aortic dissection is the separation of the aortic media induced by blood flow [14]. And the development of aortic dissection may induce the occurrence of the aortic aneurysm [15]. However, the specific molecular mechanism of aortic dissection is still unclear.…”
