Acute arterial complications associated with total hip and knee arthroplasty

Abstract: In this series, risk for arterial injury associated with THA and TKA was remarkably low. Nonetheless, even at a high-volume orthopedic hospital, acute arterial injury was not recognized on the day of surgery in about half of patients. Judicious use of preoperative arteriography and aggressive revascularization are critical to achieving limb salvage. Simple arterial thrombectomy to treat ischemic complications of THA and TKA is rarely sufficient.

“…Peripheral fibrosis secondary to previous surgery, infection, and advanced arthrosclerotic disease are all risk factors for vascular complications during total hip replacement 9,12 . Reports in the literature indicate an average frequency of 0.2 to 0.3% [12][13][14][15][16] with prevalence varying from 0.08% for a single group of hip prostheses to 0.67% 15,16 Complications can be delayed as in the case of pseudo-aneurysms and arteriovenous fistulas, or can result in immediate hemorrhage sometimes with compressive hematomas and/or ischemia from arterial injury, traumatic arterial tearing or mobilization of atheromatous plaque 17 .…”

“…As this population ages, more pre-existing medical conditions will be present in the patient base, potentially increasing the surgical risk [2][3][4]. The risk of vascular injury is increased in patients with pre-existing vascular disease symptoms such as sensory disturbances, claudication, atherosclerosis, or decreased distal pulses [5][6]. Total hip arthroplasty is considered a safe, elective procedure but is not without fatal complications [7][8][9].…”

“…Compromise of the vascular supply is a complication with serious consequences. Specifically, acute arterial thrombosis is a life-and limb-threatening injury, and although rare, can occur during THA [5][6]. Arterial complication associated with THA are reported to be 0.08% [5].…”
