Acute bacterial otitis externa: efficacy and safety of topical treatment with an antibiotic ear drop formulation in comparison to glycerol treatment

Abstract: This study proves that PS is an effective and well-tolerated drug, showing results superior to glycerol, especially in patients with a longer pre-existing condition before therapy. The absence of a group treated with another established antibiotic is a limitation of this trial.

“…Drug-induced haemolytic anaemia (DIHA) and thrombocytopenia (DIT) are common adverse effects associated with antibiotics [66]. Reports suggested that the haemolytic effects of antimicrobial peptides tyrocidine A and gramicidin S, limited their use as topical agents [67,68]. Ceftriaxone, a third-generation cephalosporin was also reported to cause haemolysis, attracting advise for restricted administration of the drug [69].…”

“…For the present study we selected anhydrous glycerol for optical clearing as glycerol is a widely used component of commercially marketed eardrops for softening and dispersing cerumen (19), is used at 1,000 mg/mL concentration to treat acute otitis media (20), and is a component in nasal sprays (21). The long and widespread use of glycerol in these applications gives some confidence in glycerol's general safety and capacity to be tolerated when used in the external ear canal.…”

“…Tyrocidine is a mixture of cyclic decapeptides that due to hemolytic properties exclusively are used as a topical agent [ 48 ]. Tyrocidine A and gramicidin S are broad-spectrum cyclodecapeptides that also only are applicable for topical use due to hemolytic side effects [ 49 , 50 ]. The cyclic lipopeptides polymyxin B and polymyxin E (also known as colistin) have been used for treatment of Gram-negative infections, and currently they constitute the last-line therapy for MDR Gram-negative infections [ 51 ].…”
